ECCO-ESGAR monitoring recommendations: Response to treatment in active ulcerative colitis should be determined by clinical parameters, endoscopy, and laboratory markers such as C-reactive protein (CRP) and fecal calprotectin; in UC patients who clinically respond to therapy, mucosal healing should be determined endoscopically or by fecal calprotectin approximately 3 to 6 months after treatment initiation.

These coverage criteria apply only to individuals aged 18 years or older. The policy’s covered uses are limited to two adult indications: (1) fecal calprotectin testing to assist in the differential diagnosis of inflammatory gastrointestinal disease (e.g., IBD) versus non‑inflammatory disease (e.g., IBS), and (2) fecal calprotectin testing to assess response to therapy, detect relapse, or monitor conditions such as inflammatory bowel disease. Tests performed in individuals under age 18 are excluded from coverage under this policy.

Rapid point‑of‑care fecal calprotectin assays (for example, CalproSmart™) are available internationally as point‑of‑care tests; however, these rapid devices have not been approved for use in the U.S. by the FDA. FDA‑cleared/approved assays and validated laboratory‑developed tests (LDTs) are the methods described for clinical use in the U.S.

Fecal calprotectin testing for any situations other than the two covered adult indications described above (differentiating IBD from non‑inflammatory disease, or assessing response/relapse/monitoring of IBD) does not meet coverage criteria due to insufficient published evidence of clinical benefit.

Clinical Evaluation Context

Fecal calprotectin should be used as part of a stepwise clinical evaluation and in combination with symptoms, other biomarkers (e.g., CRP, fecal lactoferrin), and endoscopy or imaging when appropriate. Indeterminate or borderline results may warrant serial monitoring or further diagnostic assessment per specialty guidance.

Use alongside clinical assessment, other biomarkers, and endoscopic/imaging evaluation as indicated.
Serial calprotectin testing may be considered for indeterminate results or mild symptoms per AGA guidance.

Calprotectin is a neutrophil‑derived calcium‑ and zinc‑binding protein that is excreted in feces during intestinal inflammation. Measurement of fecal calprotectin is used as a noninvasive biomarker of mucosal inflammation to help distinguish inflammatory bowel disease from non‑inflammatory conditions and to monitor treatment response or detect relapse. Assays for fecal calprotectin include FDA‑cleared or approved in vitro immunoassays (for example, the DiaSorin LIAISON® Calprotectin and other automated assays) and laboratory‑developed tests validated under CLIA; rapid point‑of‑care devices exist internationally but are not FDA‑approved for use in the U.S.
