Coverage Criteria for Salivary Hormone Testing

Initial diagnostic testing for Cushing syndrome

Covered when ALL of the following are met

Late-night salivary cortisol (LNSC) for evaluation of Cushing syndrome: Testing is covered for individuals with signs and symptoms of Cushing syndrome.At least two LNSC measurements should be obtained; diagnosis typically requires at least two abnormal first-line tests (e.g., LNSC and 24‑hour urinary free cortisol).

Endocrine Society recommends LNSC as a first-line diagnostic test for Cushing syndrome and advises obtaining at least two late-night salivary cortisol measurements; two abnormal first-line tests establish the diagnosis.

Cushing syndrome diagnostic use: Use of late-night salivary cortisol measurements for diagnosis of Cushing syndrome when performed as recommended by the Endocrine Society.>=2 late-night measurements

NAMS and Endocrine Society position statements discourage use of salivary testing to adjust hormone therapy due to lack of standardization, quality control, and demonstrated biological meaning.

Salivary and urine hormone testing to determine dosing or to monitor menopausal hormone therapy (including compounded bioidentical preparations) is not recommended. Position statements from the North American Menopausal Society and the Endocrine Society state that salivary testing is "inaccurate and unreliable" for this purpose and that salivary and urine testing to determine dosing are "unreliable and not recommended." These societies note marked differences between serum, saliva, and tissue levels and cite lack of standardization and reference ranges as key limitations.

Broad salivary hormone panels offered to guide individualized hormone replacement therapy or to monitor therapy are considered not medically necessary. Scientific review and society guidance conclude that salivary assays for many sex steroids show poor correlation with serum, have large within‑patient variability, and lack demonstrated biological meaning; therefore such panels do not provide reliable information to direct individualized dosing or clinical decisions.

Use of salivary hormone assays to adjust or dose hormone therapy is discouraged and may be denied. Major professional societies (Endocrine Society, NAMS, ACOG/ASRM) advise against using salivary testing to assess or monitor hormone therapy because assays are not standardized, lack independent quality control and accepted reference ranges, and the results are not shown to correlate with symptoms or treatment response.

When ordering or documenting salivary hormone testing, identify the specific assay and performing laboratory, and document that the test method and reference ranges have been validated. Note many salivary assays are laboratory-developed tests (LDTs) regulated under CLIA but not FDA-cleared; include the lab name, assay type (e.g., immunoassay vs LC-MS/MS), and the diagnostic cutoffs used in the report.

Background and Rationale

Salivary testing was developed as a noninvasive method to measure the free (unbound) fraction of steroid hormones in an ultra‑filtrate of blood. Cortisol diffuses readily into saliva and LNSC correlates with serum free cortisol, supporting its use in evaluating hypercortisolism such as Cushing syndrome. In contrast, salivary measurements for many sex steroids (estrogens, progesterone, and androgens) show poor correlation with serum and substantial intra‑patient variability, especially with exogenous hormone administration. These analytic and biologic limitations—together with lack of assay standardization and reference ranges—provide the rationale for restricting coverage to LNSC for suspected Cushing syndrome and for not covering salivary testing for broad hormone panel uses or for dosing menopausal hormone therapy.
