When BTMs may be useful

Guideline-based contexts where BTMs may be appropriate

Appropriate uses: Use BTMs for: predicting rate of bone loss; early assessment of treatment response (often within 3–6 months); assessing adherence/persistence to therapy (serum CTX for antiresorptives, P1NP/PINP for anabolics); evaluating offset of drug effect during a bisphosphonate holiday or after stopping denosumab; assessing disease activity or relapse in Paget disease; or when BMD measurement is unavailable or delayed.

Preferred reference markers are PINP and CTX; BALP and TRACP‑5b preferred in CKD per guideline groups.

Routine use discouraged

Limitations and recommendations against routine use

Routine use not recommended by some societies: Several organizations (including NAMS, ESCEO consensus groups, and RACGP) state BTMs should not be used routinely to diagnose osteoporosis or in the routine evaluation of all patients; use is recommended selectively by specialists or in specific contexts due to analytical variability and limited individual predictive value.

Routine measurement for all osteoporosis patients is generally not recommended.

Chronic kidney disease

CKD and special-population considerations

CKD guidance: In patients with CKD stages G3a–G5D, prefer measurement of serum PTH or bone‑specific alkaline phosphatase (BALP) to evaluate bone disease; do not routinely measure collagen synthesis/breakdown BTMs (e.g., PINP, CTX) for bone turnover assessment in CKD patients.

BALP and TRACP‑5b may be useful as they are less affected by renal failure.

Indications for targeted use

Covered when used in specialist contexts and for specific clinical purposes:

Specialist monitoring and specific indications: Measurement confined to specialist practice or when used to monitor medication adherence or effectiveness, to evaluate secondary causes of bone loss, or to assess metabolically active Paget disease.

Markers may be useful within months of initiating therapy to assess response; document rationale and timing when ordered.

Abnormal result action criteria

Situations that require additional evaluation before attributing results to postmenopausal osteoporosis:

High marker levels prompt alternative diagnosis workup: Very high values (>1.5× upper reference limit) or persistent elevations (for example up to 6 months after fracture) should prompt evaluation for other causes such as hyperparathyroidism, hyperthyroidism, Paget disease, malignancy (including myeloma), or advanced renal failure before attributing results to postmenopausal osteoporosis.>1.5× upper reference limit

High values are not typical of postmenopausal osteoporosis.

Measurement of bone turnover markers to monitor teriparatide therapy DOES NOT MEET COVERAGE CRITERIA. This exclusion is explicit: BTMs are not covered for the purpose of monitoring teriparatide treatment in individuals with osteoporosis.

Multiple professional societies state that bone turnover markers cannot be used to diagnose osteoporosis. NAMS and the ESCEO consensus both explicitly note that BTMs are not diagnostic for osteoporosis and have limited individual predictive value, so measurement for diagnostic purposes DOES NOT MEET COVERAGE CRITERIA.

Routine use of bone turnover markers for population-level fracture risk assessment or in primary care is not routinely recommended due to substantial biological and analytical variability and lack of assay standardization. Guideline bodies (for example, RACGP) note that BTMs may be useful in population studies but their utility for individual risk assessment in general practice is limited; measurement should be confined to specialist practice or specific clinical scenarios.

Use of BTMs as a diagnostic test for osteoporosis and for the diagnosis or routine management of other high bone‑turnover conditions is considered not medically necessary. The policy and referenced societies indicate these indications DO NOT MEET COVERAGE CRITERIA because available evidence does not establish that such testing is required or beneficial for individual patient diagnosis or routine management.

Routine measurement of BTMs for all patients with osteoporosis is not recommended and would generally be unnecessary outside defined specialist or monitoring contexts. Professional guidance indicates markers are best used selectively (for example, to assess adherence or early treatment response in specialist care) rather than as a routine test for every osteoporosis patient.

Routine, non‑specialist ordering of bone turnover markers for general osteoporosis screening or as a substitute for bone mineral density measurement is not supported. RACGP and related guidance recommend that measurement be confined to specialist practice or defined indications because variability, limited individual predictive value, and lack of standardization reduce utility in general screening.

Tests ordered for purposes that do not meet coverage criteria are subject to denial. BTMs submitted as a diagnostic test to establish osteoporosis or for management of non-osteoporosis high bone-turnover conditions are examples of services that DO NOT MEET COVERAGE CRITERIA and may be denied.

BTMs submitted to diagnose osteoporosis (not covered).
BTMs submitted for diagnosis/management of other high bone-turnover conditions (not covered).
Claims lacking documentation of the clinical rationale described in this policy may be denied.

Documentation Required

CKD measurement caution

In patients with CKD stages G3a–G5D, routine measurement of collagen-derived BTMs (e.g., PINP, CTX, cross-laps, pyridinoline, deoxypyridinoline) is discouraged. Consider using serum PTH or bone‑specific alkaline phosphatase when evaluating bone disease in CKD per KDIGO guidance.

KDIGO suggests not to routinely measure collagen synthesis/breakdown BTMs in CKD G3a–G5D.

Very high marker levels (>1.5× upper reference limit)

Very high BTM values (greater than 1.5 × the upper reference limit) are not typical of postmenopausal osteoporosis and should prompt evaluation for alternative causes of elevated turnover (for example: recent fracture, hyperparathyroidism, hyperthyroidism, Paget disease, malignancy including myeloma, or advanced renal failure). Document steps taken to evaluate alternative causes.

Investigate alternative causes when BTM > 1.5× upper reference limit.
Note that BTMs may remain elevated for up to six months after a fracture.
