Emergency Scene Transport Medical Necessity

Covered when ALL of the following are met

Primary condition: The time needed to transport by ground, or instability of ground transport, poses a threat to the individual's survival or seriously endangers the patient's health.

Required

Support conditions: Either the point of pickup is inaccessible by ground OR great distances or other obstacles (e.g., traffic) would prevent timely arrival at the nearest appropriate facility.

One of these must be met in addition to the primary condition

Interfacility Transfer Medical Necessity

Covered for interfacility transfer when ALL of the following are met

Interfacility transfer criteria: Ground ambulance transport would endanger the patient's health AND the sending hospital lacks the necessary capabilities AND the receiving hospital is the nearest facility with appropriate services.

Transport of Deceased Individuals

Deceased patient transport: Air and water ambulance services for deceased individuals are medically necessary when the above criteria were met and the individual was pronounced dead at the scene or while en route; if death occurred prior to ambulance arrival but after dispatch, services may be considered medically necessary. Mileage is not considered medically necessary when the individual expired prior to ambulance arrival or at the scene.

Operational: mileage exclusion applies as stated

The following transports are explicit exclusions and are not considered medically necessary under this policy: transport requested solely because of patient or family preference for a particular hospital or physician; and transport to non-acute destinations such as a nursing facility, a physician’s office, or the patient’s home.

For mileage and transport billing, mileage is not medically necessary when the individual expires prior to ambulance arrival or at the scene. Additionally, transport from a hospital solely because the patient or family prefers a different hospital is considered not medically necessary, as are transports to non-acute care settings (nursing facility, physician office, or home).

Prior Authorization and Medical Appropriateness

Prior authorization requirements are determined by the member's benefits and the provider's network agreement with Capital BlueCross. Providers must obtain any required prior authorization before air or water ambulance transport when member plan terms or network contracts require it.

Required Clinical Documentation

Documentation must support that ground transport time or instability poses a threat to survival or seriously endangers the patient's health, or that the transferring hospital lacks required specialized services and the receiving hospital is the nearest facility able to provide needed care.

Clinical notes describing the patient’s condition and physiologic instability.
Time/distance estimates comparing ground vs air/water transport.
Evidence the transferring facility lacks required services (e.g., burn center, cardiac/trauma/critical care).
If deceased, documentation that criteria were met and time of death relative to ambulance arrival/dispath.

Denial Risk

Triggers for Denial or Noncoverage

Requests may be denied or considered not medically necessary when ground transport would not endanger the patient's health, when transport is requested solely for patient or family preference, or when transport is to a non–acute care facility (e.g., nursing facility, physician office, member’s home).

Transfers for patient/family preference to a specific hospital or physician are not medically necessary.
Transport to non-acute care destinations is not medically necessary.
Mileage/transport charges for individuals who expired prior to ambulance arrival or at the scene are not medically necessary; limited exceptions apply if death occurred after dispatch and criteria were met.

Background

Air ambulance involves rotary-wing (helicopter) or fixed-wing (airplane) aircraft specifically designed and equipped to transport the sick or injured; water ambulance uses boats similarly equipped. These services must carry customary patient care equipment and safety/lifesaving supplies. Crews are required to consist of at least two attendants, with one attendant qualified to provide the medical care required during transport. Air and water ambulance services are intended for situations where ground transport is unsafe or impractical or when specialized capabilities are required at the receiving facility.
