This policy defines medical necessity criteria and required documentation for orthognathic surgery for Capital Blue Cross products. Orthognathic surgery (typically performed with orthodontic care) is covered when medically necessary for correction of significant congenital facial skeletal deformities, restoration after trauma/tumor/infection, or treatment of malocclusion that causes significant functional impairment (for example mastication dysfunction, speech abnormality, persistent pain refractory to conservative therapy, or airway obstruction such as OSA). Procedures performed primarily for psychological, cosmetic, orthodontic-only reasons, to prevent future problems without established functional impairment, or where high risk of recurrence exists are considered investigational.

Medical Necessity Criteria

General documentation requirements

Documentation should include ALL of the following:

Required documentation: Medical history and physical examination
Required documentation: Description of specific anatomic deformities
Required documentation: Previous management of functional medical impairments
Required documentation: Details of any failed non-surgical/conservative therapies
Required documentation: Dental molds, photographs, and x-rays (Ortho-Panores, cephalometric) including measurements and angles

Prerequisite orthodontic therapy

If preparatory orthodontics performed:

Orthodontic completion: Any preparatory orthodontic therapy must be completed prior to surgery
Post-orthodontic persistence: The condition for which surgery is considered must persist and be documented after orthodontic therapy (the original problem must survive the orthodontics)

Medically necessary indications

Orthognathic surgery may be considered medically necessary when ANY of the following primary reasons are met:

Congenital deformity: Correction of significant congenital deformity (examples include LeFort III for Crouzon, Pfeiffer, cleft lip/palate, Treacher Collins, Apert; mandibular surgery for congenital micrognathia causing respiratory obstruction; Pierre Robin sequence; maxillary deformity from cleft)
Restoration after acquired conditions: Restoration following trauma, tumor, degenerative disease, or infection (other than mild gingivitis)
Malocclusion with functional impairment: Treatment of malocclusion that contributes to one or more significant functional impairments (see functional impairment node)

Functional impairments qualifying under malocclusion

For malocclusion-related surgery, MUST meet at least ONE of the following functional impairments AND the anatomic requirements:

Mastication dysfunction with persistent difficulty swallowing, choking, or adequately chewing food
Speech abnormality
- Speech - layperson notice: Speech deficit is noticeable to a layperson or primary care physician and significantly impairs the individual's ability to communicate
- Speech - etiology: Professional speech evaluation indicates the speech deficit is the direct result of an anatomical jaw abnormality and is not amenable to speech therapy
- Speech - expected improvement:

Anatomic requirements (must meet ANY one of these when functional impairment due to malocclusion present)

ONE of the following anatomic criteria is required:

Anteroposterior discrepancies
- Incisor relationship - overjet: Maxillary/Mandibular incisor relationship abnormal: Horizontal overjet of +5 mm or more+5 mm or greater

- Incisor relationship - zero/negative: Maxillary/Mandibular incisor relationship abnormal: Horizontal overjet of zero to a negative valuezero to negative

Additional covered clinical situations

Orthognathic surgery may also be considered medically necessary for ANY of the following:

Significant dental trauma (e.g., shearing or fractured teeth) related to malocclusion with supplied information indicating severity and duration
Persistent pain attributed primarily to qualifying facial skeletal discrepancies and refractory to at least 3 months of conservative therapy (e.g., medication, physical therapy)>= 3 months
Airway obstruction/OSA: Airway obstruction (such as obstructive sleep apnea) that is BOTH confirmed by appropriate sleep study AND persisting despite failed conservative treatments (e.g., CPAP, oral appliance) and/or less invasive surgical procedures

Investigational / Not medically necessary indications

Orthognathic surgery is considered investigational when performed for ANY of the following:

Cosmetic/psychological/orthodontic primary: Performed primarily for psychological, cosmetic, or orthodontic purposes
Preventive/future problems: Surgery designed to eliminate potential future problems where functional impairment is not yet established
High recurrence risk/insufficient evidence: Surgery where significant risk of recurrence of symptoms or structural abnormalities exist (insufficient evidence of benefit)

Providers must submit documentation to establish medical necessity for orthognathic surgery. Orthognathic surgery surgically corrects facial skeletal deformities (maxilla, mandible, chin) to restore proper anatomic and functional relationships and is often performed in conjunction with orthodontics. Indications include congenital malformations present at birth or evident with growth (e.g., cleft-related maxillary deformity, syndromic craniofacial conditions), sequelae of trauma, tumor, degenerative disease or infection, and malocclusion causing abnormal speech, chewing/eating difficulties, respiratory problems, or temporomandibular joint dysfunction.
