Medically Necessary Criteria

General anesthesia, associated medical services, and facility admission may be considered medically necessary when ANY of the following apply:

Medically significant condition: Individual (adult or child) has a medically significant condition (e.g., unstable heart disease, severe asthma, severe chronic obstructive pulmonary disease, seizures, hemophilia)

Supported by policy statement

History of severe postoperative complications: Individual (adult or child) has a history of severe postoperative complications following oral or dental surgery

High probability of complications due to procedure complexity: Scheduled for dental or oral surgical procedures such as bony impacted tooth extractions with high probability of complications (including impactions high in the upper jaw that may impinge on the sinuses or deep lower jaw impactions)

Includes complex bony extractions where facility admission may be required

Young age: Individual who is 7 years or younger<=7 years

Aligns with Act 94 eligibility criteria

Developmental disability: Individual who is developmentally disabled (conditions such as ADHD, autism spectrum disorder, cerebral palsy, hearing or vision impairment, intellectual disability, learning disability, or other developmental delays)

Per Act 94: when successful treatment under local anesthesia cannot be expected and superior result is expected under general anesthesia
