- Trigger point injections with anesthetic and/or corticosteroid are for the treatment of myofascial pain syndrome when criteria in Blue KC Medical Policy 2.01.03 are met.
- Trigger point injections are investigational for all other indications (including complex regional pain syndrome, abdominal wall pain, fibromyalgia).
Ultrasound guidance for trigger point injections is considered investigational.

Trigger point injections with anesthetic and/or corticosteroid may be paid when used to treat myofascial pain syndrome only when criteria in Blue KC Medical Policy 2.01.03 are met; injections are investigational for all other indications. Ultrasound guidance for trigger point injections is considered investigational. No more than 4 injections may be given in a 12‑month period. When a trigger point injection is performed the same day as an E/M service, the E/M will be bundled and denied unless documentation supports a significant, separately identifiable E/M service (report Modifier 25 if applicable).

Documentation must include muscles injected (named), medication, needle size, agent (drug/substance), strength, and quantity injected.
Code selection is based on the number of muscles injected, not the number of injections performed.

DefinitionTrigger points are tight bands of muscle fibers that can’t relax, creating painful, tender ‘knots’.

Procedure descriptionTrigger point injections involve injecting a local anesthetic or corticosteroid into a ‘knot’ of muscle tissue to relieve pain and muscle spasms.
