Covered when ALL of the following conditions and operational rules are met:

Medical necessity
- Trigger point injections with anesthetic and/or corticosteroid are for the treatment of myofascial pain syndrome when criteria in Blue KC Medical Policy 2.01.03 are met.
- Trigger point injections are investigational for all other indications (including complex regional pain syndrome, abdominal wall pain, fibromyalgia).
- Ultrasound guidance for trigger point injections is considered investigational.
