Coverage stance for Serious Adverse Events and Hospital Acquired Conditions

Coverage stance and rules for SAEs/HACs

Non-payment applies when the SAE/HAC is identified on claim review or via POA indicators and matches CMS/NQF lists.

Do not reimburse providers for CMS-defined Hospital Acquired Conditions (HACs) when the condition was not present on admission and is identified on claim review; examples include foreign object retained after surgery; air embolism; blood incompatibility; Stage III/IV pressure ulcers; falls and trauma (fractures, dislocations, intracranial injuries, crushing injuries, burns, other injuries); manifestations of poor glycemic control (diabetic ketoacidosis, nonketotic hyperosmolar coma, hypoglycemic coma, secondary diabetes with ketoacidosis or hyperosmolarity); catheter-associated urinary tract infection; vascular catheter-associated infection; specified surgical site infections (including mediastinitis after CABG, infections after bariatric and certain orthopedic procedures, CIED infections); DVT/PE after certain orthopedic procedures (total knee, hip replacement); and iatrogenic pneumothorax with venous catheterization.

Do not reimburse providers for designated 'Never Events' (Serious Reportable Events) including: surgery performed on the wrong body part; surgery performed on the wrong patient; and wrong surgical procedure performed.

Blue KC Quality Management reviews monthly system-generated claim reports for codes that match CMS Never Event indicators; hospital or ancillary provider documentation may be requested for review, and identified occurrences are shared with Hospital and Ancillary Contracting Departments. Members are held harmless for any reimbursement related to Serious Adverse Events.

Present on Admission (POA) indicators must be used to determine whether the condition existed at the time of inpatient admission; HACs are not reimbursed when they were not present on admission and are identified via POA reporting or claim review.

Outpatient and inpatient modifiers are used to assist identification of never events and related claims (examples include outpatient modifiers PA/PB/PC and inpatient modifiers MX/MY/MZ).

When such codes are identified, hospital or ancillary provider documentation may be requested by a Blue KC Quality Auditor for review alongside other claims data to determine if a never event occurred. Identified occurrences are shared with Hospital and Ancillary Contracting Departments; members are held harmless for any reimbursement related to Serious Adverse Events.
