Postsurgical outpatient VTE prophylaxis coverage criteria

Covered when ALL of the following are met:

Device use must be determined to be medically necessary for postsurgical outpatient VTE prophylaxis per Blue KC Medical Policy 1.01.28.
ALL of the following
- Examples (ONE or MORE of the following)
  - After major orthopedic surgery (total hip arthroplasty, total knee arthroplasty, hip fracture surgery)
  - After major non-orthopedic surgery or other orthopedic procedures in individuals who are at moderate or high risk of VTE
Facility setting exclusion
Use of limb compression devices in facility settings (ambulatory surgery center POS 24 or hospital POS 22/21) is considered inclusive to the surgical procedure and will not be separately reimbursed.
Duration limit
Postsurgical outpatient use for VTE prophylaxis beyond 30 days may not be reimbursed.

Prior authorization / reimbursement conditions for postsurgical outpatient limb compression devices

Postsurgical outpatient limb compression device use is reimbursable only when determined to be medically necessary and when the individual has a contraindication to pharmacologic VTE prophylaxis; see Blue KC Medical Policy 1.01.28 for clinical indications. Device use performed in facility settings (ambulatory surgery center POS 24 or hospital POS 22/21) is considered inclusive to the surgical procedure and will not be separately reimbursed. Reimbursement for postsurgical outpatient VTE prophylaxis beyond 30 days may not be provided.

Reimbursable when medically necessary and per Blue KC Medical Policy 1.01.28 (clinical indications).
Reimbursable when the individual has a contraindication to pharmacologic prophylaxis — examples include after major orthopedic surgery (THA, TKA, hip fracture) or after major non-orthopedic surgery in patients at moderate/high VTE risk.
Not separately reimbursed when provided in facility settings: POS 24 (ambulatory surgery center) or POS 22/21 (hospital).
Postsurgical outpatient use beyond 30 days may not be reimbursed.
