NCCI Edits (National Correct Coding Initiative) and Modifier Use
Defines Blue KC's expectations for applying NCCI procedure-to-procedure (PTP), medically unlikely edits (MUE), and add-on code edits, including allowed modifiers and documentation requirements; applies to claims submitted to Blue KC across all lines of business and provider types.
Blue KC enforces Type I add-on code edits but does not currently enforce Type II or III add-on code requirements.
Primary and add-on codes must be billed on the same day by the same provider, with a stated exception for initial drug administration per vascular access site during observation stays.

Additional anatomical and global modifiers that may override an NCCI conflict were added to the list of allowed modifiers.
