Coverage and billing rules

Services must be billed on one claim per session under the mother's name; claims billed under the baby will be denied.

Services may be provided in a hospital, clinic, breastfeeding center, provider office, or the member's home; the provider should indicate the appropriate place of service code on the claim.

Lactation support services may be provided by qualified health care professionals with specific training in lactation, including physicians and non-physicians (e.g., registered nurses, physician assistants, nurse practitioners, and others acting within the scope of their license or certification).

When the service is preventive for the mother (risk factor reduction or preventive counseling), use Preventive Medicine Counseling codes (CPT 99401-99404; group codes 99411-99412 as applicable).

Lactation support services — definition and scope

DefinitionEducation, counseling, or behavioral interventions to improve breastfeeding outcomes, such as promoting breastfeeding, helping with latch and positioning, managing lactation problems, and counseling on return to work or school.

Scope of servicesMay include assistance with latch and positioning, managing lactation problems, counseling on returning to work/school, addressing other breastfeeding concerns, and coordination with other health care providers.

ProvidersMay be provided by qualified health care professionals with lactation training (physicians and non-physicians such as registered nurses, physician assistants, nurse practitioners, or others acting within scope of license).

SettingsServices may be provided in hospital, clinic, breastfeeding center, provider office, or the member's home (place of service must be indicated on the claim).
