Indication-specific coverage

IBD indication: Individuals with inflammatory bowel disease (IBD): Drug and/or antibody concentration testing for anti‑TNF therapies, vedolizumab therapy, or ustekinumab therapy may be reimbursed once every two weeks.
Non‑IBD inflammatory conditions (exclusion): Individuals with conditions other than IBD (e.g., spondyloarthritis, rheumatoid arthritis, psoriatic arthritis, and psoriasis): Drug and/or antibody concentration testing for anti‑TNF therapies may not be reimbursed.

Other situations and specific drugs (exclusion): For all other situations not addressed above, measurement of serum drug levels and/or antibodies may not be reimbursed for any of the following drugs (alone or as a combination test): adalimumab; certolizumab; etanercept; golimumab; infliximab; infliximab‑dyyb; infliximab‑abda; rituximab; ustekinumab; vedolizumab.

Reimbursement authorization summary for immunopharmacologic monitoring

For individuals with inflammatory bowel disease (IBD), drug and/or antibody concentration testing for anti‑TNF therapies, vedolizumab, or ustekinumab may be reimbursed when performed once every two weeks; coverage remains subject to the member’s benefit. Drug and/or antibody concentration testing for anti‑TNF therapies for conditions other than IBD (for example, spondyloarthritis, rheumatoid arthritis, psoriatic arthritis, and psoriasis) will not be reimbursed. Measurement of serum drug levels and/or antibodies may also not be reimbursed in other situations or for the listed drugs (adalimumab, certolizumab, etanercept, golimumab, infliximab and its biosimilars, rituximab, ustekinumab, vedolizumab). Billing should use the policy’s referenced test codes/descriptions as applicable and follow member benefit verification prior to testing.
