- BNP or NT-proBNP to diagnose heart failure in individuals presenting with dyspnea
- BNP or NT-proBNP to establish disease severity in individuals with chronic heart failure
  Reimbursed up to 4 times per year in the outpatient setting

ALL of the following

NONE of the following
- Qualitative measurement of cardiac troponin (troponin T or I) in the outpatient setting
- Measurement of the following biomarkers for diagnosis/prognosis of MI: AST/SGOT; CKMB; CK; creatine kinase isoenzymes; LD/LDH; myoglobin
- Measurement of additional biomarkers: copeptin; troponin C; C-reactive protein; heart-type fatty acid binding protein (H-FABP); any other cardiac biomarkers not listed as covered
- Analysis of ST2 and/or its isoforms (e.g., Presage ST2) in the outpatient setting
Cardiac biomarker measurements performed in outpatient settings not capable of performing adequate clinical MI evaluation (e.g., independent laboratory or physician office) are not reimbursed

Policy limitQuantitative troponin (T or I) may be reimbursed up to 4 times within the first 72 hours following initial presentation for individuals with signs/symptoms of acute coronary syndrome when tested at an outpatient facility capable of adequate MI evaluation.

Setting requirementReimbursement applies when testing is performed at an outpatient facility capable of performing an adequate clinical MI evaluation; testing in independent labs or physician offices not capable of adequate MI evaluation may not be reimbursed.

Quantitative troponin (T or I) testing performed for individuals presenting with signs/symptoms of acute coronary syndrome may be reimbursed up to four times during the first 72 hours after initial presentation when testing is done at an outpatient facility capable of performing an adequate clinical MI evaluation. Document the patient’s signs/symptoms of acute coronary syndrome and the setting of testing; tests performed in outpatient settings that are not capable of adequate MI evaluation (for example, independent laboratories or physician offices) are not reimbursed and may be denied.

Reimbursed frequency: up to 4 quantitative troponin measurements within the first 72 hours following initial presentation.
Coverage condition: patient must present with signs/symptoms of acute coronary syndrome and testing must occur at an outpatient facility capable of adequate clinical MI evaluation.

Acute examplesTroponin and creatine kinase MB isoenzyme (CKMB) are commonly used in the acute clinical setting to detect MI.

Chronic/example roleMarkers such as suppression of tumorigenicity 2 (ST2) can serve long-term as markers of cardiomyocyte stress and fibrosis for risk stratification.

inv-07: Acute coronary syndrome (ACS) signs/symptoms

Typical ischemic chest pain featuresIschemic chest pain often with radiation to an upper extremity, both arms, lower jaw, neck, back (interscapular region), or upper abdomen; may be described as squeezing, pressure, crushing, burning, or fullness.

Associated autonomic/constitutional featuresDiaphoresis, nausea, vomiting, shortness of breath, dizziness, lightheadedness, clamminess, and fatigue often accompany ischemic chest pain.

Atypical presentationsAtypical signs include dyspnea alone, weakness, nausea/vomiting, epigastric pain or discomfort, palpitations, syncope, or cardiac arrest—these should also prompt consideration of ACS.
