Such services are subject to the member’s Contract/Certificate of Coverage and may be subject to deductibles, copays, and/or coinsurance rather than the 100% preventive benefit.

When a provider orders or performs services during a preventive visit that are treatment-related or diagnostic (for example, testing for symptoms or management of a known condition), those services do not qualify as preventive and will be processed under the plan’s standard coverage rules. In-network routine preventive services and the related office visit for routine preventive care are covered at 100% when billed with a primary diagnosis of preventive. Most services must be billed with a primary preventive diagnosis to be covered at 100%. Routine preventive care services are subject to the terms, conditions and limitations of the member's Contract/Certificate of Coverage; not all plans cover all preventive services at 100%—consult the Certificate of Coverage for details.

Services ordered or provided during a preventive visit that are diagnostic or treatment-related (e.g., evaluation/treatment of an existing condition or symptomatic complaint) may be subject to deductibles, copays and/or coinsurance.
Providers should use current diagnosis and procedure codes when submitting claims to ensure correct adjudication.
