Point-of-sale hard edit and denial triggers

Point-of-sale (POS) hard edits and denials are applied at the pharmacy when CMS-specified safety thresholds are met. Claims may be denied at POS when hard-edit conditions occur; pharmacists may be permitted to override specific soft edits but cannot override hard edits without an approved prior authorization (PA) or exception.

Cumulative MME POS edits: Soft edit at >= 90 MME with ≥ 2 prescribers (pharmacist may override). Hard edit at >= 200 MME requires an approved prior authorization exception to process. Members in hospice, with cancer or sickle cell diagnosis, or in long-term care are excluded from cumulative MME triggers.
Opioid-naive supply limit: Hard edit limits opioid-naive patients (no paid opioid claims in past 180 days) to a 7-day supply. Attempts to fill >7-day supply will reject at POS and require prior authorization. Residents of long-term care, patients with cancer or sickle cell disease, and hospice patients are excluded.
Early refill, therapeutic duplication, and maximum dose edits: Edits detect early refills, therapeutic duplication, and doses exceeding FDA maximums (including combination products where acetaminophen could exceed 4 g/day). These edits may be soft or hard depending on the specific trigger; pharmacist override is allowed for soft edits, while hard edits require PA.
Long-acting opioid overlap and opioid–benzodiazepine overlap: Soft edits identify overlapping claims for multiple long-acting opioids and for concurrent opioid and benzodiazepine use (any overlap in days supply). These soft edits may be overridden by a pharmacist or with prior authorization; long-term care, cancer, sickle cell, and hospice patients are excluded from POS logic for these edits.

When a hard edit denies a claim at POS, an approved prior authorization or exception request is required to allow processing. Pharmacists should follow plan procedures for notifying prescribers and initiating PA requests. Exemptions apply for members in hospice, with cancer or sickle cell disease, or residing in long-term care.

Prescriber communication and response for retrospective review

If retrospective review identifies potential overutilization, Blue Medicare Advantage will contact prescribers by written inquiry and follow-up phone call. Prescribers are expected to respond promptly to facilitate case management; if the plan determines the member is at risk, a beneficiary-level edit (lock-in) may be placed with 30-day notice and opportunity to request coverage determination or appeal.

Prescribers receiving an Opioid Overutilization Monitoring Program fax should respond promptly to support case management.
If consensus is reached that utilization is not appropriate, the plan may limit the member to certain prescriber/pharmacy preferences (lock-in); members receive 30 days' notice.
Members may submit a coverage determination request to cover a higher level of opioids; if denied, members have the right to appeal and will receive written notification of outcomes.
