Medically covered and not-covered indications

Medically covered indications

Urine culture testing (with isolate identification and antibiotic susceptibilities if applicable) may be covered when ONE of the following clinical scenarios applies:

Pregnant individuals
Urine culture testing with isolate identification and susceptibilities may be covered.
Pre-procedure for mucosa-breaching urological interventions: Asymptomatic individuals undergoing urological interventions which breach the mucosa, prior to the procedure
Urine culture testing with isolate identification and susceptibilities may be covered.
Signs or symptoms of UTI/bacteriuria: Individuals exhibiting at least one sign or symptom of a possible UTI or bacteriuria (see Note 1)
Urine culture testing with isolate identification and susceptibilities may be covered.
Assessment of pyelonephritis
Urine culture testing with isolate identification and susceptibilities may be covered.

Not covered / Not medically necessary

Urine culture testing (with isolate identification and antibiotic susceptibilities if applicable) may not be covered in the following situations:

Asymptomatic UTI or asymptomatic bacteriuria not described above: Asymptomatic UTI or asymptomatic bacteriuria not described in covered indications above
Not covered.
Follow-up culture after clinical resolution: Follow-up urine culture testing for an uncomplicated UTI when the individual shows evidence of clinical resolution of infection
May not be covered.
Initial screening for asymptomatic prostatitis: As part of initial screening for asymptomatic prostatitis
Does not meet coverage criteria due to lack of supporting literature.

Denial Risk

Risk of denial for asymptomatic screening

Claims for urine culture testing for asymptomatic individuals not meeting specified covered indications (including initial screening for asymptomatic prostatitis and assessment related to prostate biopsy) may be denied as not meeting coverage criteria.

Claims for urine culture testing should be supported by documentation of a covered clinical indication (for example: pregnancy, planned mucosa‑breaching urological procedures, signs/symptoms of UTI, or assessment of pyelonephritis).
