Monitoring in diagnosed IgA-sufficient individuals: Serologic testing with IgA antitissue transglutaminase (TTG) at the first follow-up 3 to 6 months after diagnosis, every 6 months until anti-TTG normalizes, then every 12 to 24 months thereafter.3-6 months; every 6 months until normalization; every 12-24 months thereafter

DGP recommended for children under 2 years or as an alternative to TTG when appropriate.

The following tests and test formats are specifically excluded from reimbursement: rapid antigen point-of-care anti-TTG testing; panel, multiplex, or multi-analyte tests for more than two analytes used for diagnosis or evaluation of celiac disease; anti-reticulin antibody testing; and testing of stool or saliva samples for celiac disease evaluation. These exclusions apply regardless of setting and are not considered payable under this policy.

Testing for celiac disease in asymptomatic individuals who are not at increased risk for developing celiac disease is not considered reimbursable and may be denied. Providers should document signs, symptoms, or established risk factors to support medical necessity for testing.

Indications: Diagnosis and Monitoring

Diagnosis in symptomatic or at-risk individuals; monitoring in diagnosed celiac disease patients

Diagnosis and monitoring criteria — choose assays based on total IgA status and patient age; biopsy may be reimbursed for confirmation in high-risk cases.

Initial diagnostic approach for symptomatic individuals: Obtain IgA anti-TTG together with total IgA for diagnosis of celiac disease in individuals with signs or symptoms.N/A

If total IgA is low or absent, proceed to IgG-based testing (IgG EMA, IgG DGP, or IgG TTG).

Assay selection for IgA-deficient patients: When IgA deficiency is present, use IgG endomysial antibodies, IgG deamidated gliadin peptide, or IgG TTG for diagnosis and monitoring.N/A

Testing sequence based on IgA status

For IgA‑sufficient individuals: order IgA anti‑tissue transglutaminase (TTG) with total IgA. For patients confirmed IgA‑deficient: order IgG‑based assays (IgG endomysial antibodies, IgG deamidated gliadin peptide, or IgG TTG) instead of IgA‑based testing.

High-risk individuals — syndromes and autoimmune conditions associated with increased celiac prevalence

Syndromic associationsIndividuals with Down syndrome, Turner syndrome, or Williams-Beuren syndrome are considered high-risk for celiac disease.

Autoimmune conditionsPresence of autoimmune conditions (e.g., type 1 diabetes, autoimmune hepatitis) increases celiac disease risk and warrants testing.

Background

Celiac disease is an immune-mediated small intestinal enteropathy triggered by ingestion of gluten, leading to villous atrophy and malabsorption. Serologic testing — including anti-tissue transglutaminase (anti-TTG), endomysial antibodies (EMA), and deamidated gliadin peptide (DGP) antibodies — together with measurement of total IgA, are used to screen and monitor patients; intestinal biopsy is used to confirm diagnosis in selected or high-risk individuals.
