Medical necessity criteria

Covered when ALL of the following are met

Primary coverage conditions: 1) There is clinical evidence that neuropsychological testing results will have a timely and direct impact on the member's treatment plan; 2) Relevant clinical assessments and records are provided to support the indication (see documentation); 3) Indication corresponds to listed clinical reasons (e.g., traumatic brain injury, encephalitis, epilepsy with cognitive impairment, multiple sclerosis with cognitive impairment, anoxic/hypoxic injury, cerebrovascular accident, psychosis, major affective disorder, prior intracranial surgery, brain tumor, planned neurosurgery for epilepsy, head injury with loss of consciousness, confirmed neurotoxin exposure, suspected dementia).

Repeat testing to track status or recovery is subject to individual consideration and is generally not warranted.

Repeat testing to track the status of an illness or recovery progress is subject to individual case consideration and is generally not warranted. Authorization for repeat neuropsychological testing should include documentation that explains why prior test results are insufficient and how new results will have a timely and direct impact on the member’s treatment plan.

Repeat neuropsychological testing performed solely for routine tracking of illness or recovery, without case-specific clinical justification, is generally not considered medically necessary. Providers requesting repeat testing must supply clear, case-specific rationale demonstrating that the additional testing will change or inform the member’s care in a timely manner.
