PHP Initial Admission Criteria

Covered when ALL of the following are met for PHP admission:

Core admission requirements (1-6): 1) Current DSM or ICD eating disorder diagnosis consistent with symptoms (serious or disabling or exacerbation of severe/persistent disorder). 2) Marked impairments in multiple areas of daily functioning (self-care, work, problem-solving, medication compliance, etc.). 3) Not imminently dangerous to self/others and able to function outside 24-hour custodial care, though identifiable risk may be present. 4) Has or can be connected to a community-based social support network; minimal ability to set goals for developing support is required. 5) Readiness for change: capacity for at least minimal engagement in identifying treatment goals and willingness to participate. 6) Level-of-care rationale: relapse/failure in less intensive care, less intensive care judged insufficient, or stepping down from inpatient but requiring ongoing intensive interventions and daily monitoring.

Additional qualifying criteria (one or more of 7-13): At least one of the following: 7) If anorexia nervosa and weight restoration is the goal, weight is over 80% of estimated ideal weight without signs of acute medical instability; 8) Comorbid psychiatric disorders are controlled/stable enough for primary focus on the eating disorder (if suicidal ideation present, inpatient criteria must not be met); 9) Partial motivation to recover with some insight and cooperativeness; 10) Co-occurring disorders manageable at this level; 11) Need for structure for part or most of the day to eat/gain weight; 12) Purging behavior can be reduced and there are no significant medical complications; 13) Environmental stress with at least limited support and structure available.

IOP Initial Admission Criteria

Covered when ALL of the following are met for IOP admission:

IOP core requirements: 1) DSM or ICD eating disorder diagnosis consistent with symptoms. 2) Moderate symptoms causing significant distress and moderate functional impairment. 3) Not imminently dangerous to self/others and able to function outside 24-hour custodial care. 4) Has or can be connected to community-based supports (may have limited supports). 5) Level-of-care rationale: relapse/failure in outpatient care, outpatient treatment insufficient, or stepping down from inpatient/PHP and still needing stabilization.

Additional qualifying IOP criteria (one or more of 6-12): At least one of the following: 6) If anorexia nervosa and weight restoration is goal, weight is over 80% of estimated ideal weight without acute medical instability; 7) Comorbid psychiatric disorders are controlled/stable with low suicide risk; 8) Fair motivation to recover (cooperativeness, insight). 9) Co-occurring disorders manageable at this level. 10) Sufficient structure outside the program to eat/gain weight. 11) Purging behavior can be reduced without significant medical complications. 12) Adequate environmental support available.

PHP Continued Stay Criteria

Continued stay criteria (PHP):

Clinical continued stay reasons: One or more of: symptoms continue to impair multiple areas of daily functioning; impaired judgment/skill deficits place the individual at risk for further deterioration; inability to cope with significant crises or lack of necessary coping skills; continued significant risk for harm to self or others; poor insight/skills/awareness inhibiting return to baseline functioning.

Engagement and response requirements: Must also have successful engagement in the clinical process, active attendance and participation, and capacity to respond to therapeutic interventions; plus one or more of: continued need for medication monitoring/intervention, capacity to make progress in coping skills, need for support in major life crisis, continued need for risk management with ability to follow a safety plan, and commitment to a recovery-oriented discharge plan.

Initial IOP Intensity Criteria

Covered when ALL of the following intensity criteria are met:

Intensity of Service for IOP: 1. At least three (3) hours of programmed treatment services at least three (3) days per week. 2. IOP Continued Stay Criteria

Continued stay is allowed when ALL of the following are met:

The individual does not have a high likelihood of relapse to severity requiring inpatient or partial hospitalization. 3. Stability cannot be maintained with regular outpatient treatment; if weight gain is a treatment target, the member's weight is not at or over 85% of estimated ideal weight.

The policy identifies specific exclusion criteria derived from AABH guidance. Services are not appropriate for individuals who are uninterested or unable to engage in treatment planning or who decline participation; who are imminently suicidal or homicidal and require hospitalization due to insufficient impulse control or supports; who have cognitive deficits that preclude learning and behavioral change; or who have conditions not amenable to partial hospitalization (for example, certain severe manic states or social phobia not appropriate for group treatment). Programs should also exclude those whose needs are primarily social/custodial/respite in nature or who consistently cannot adhere to reasonable program expectations in ways that would harm group functioning.

Intensive Outpatient Program (IOP) services for eating disorders are not medically necessary when the documented intensity and continued stay criteria required by policy are not met. Prior authorization and clinical records must demonstrate that the member receives at least three (3) hours per day on at least three (3) days per week of active programmed treatment, that appropriate staffing, case management, and 24-hour crisis access are available, and that continued need cannot be managed in regular outpatient care; absence of these elements supports a denial as not medically necessary.

Partial Hospitalization Program (PHP) is not medically necessary when the policy's required admission, intensity, and continued-stay criteria are not satisfied.
