Examples of qualifying indications: Traumatic brain injury; anoxic/hypoxic brain injury; history of intracranial surgery; confirmed neurotoxin exposure (including cancer treatments); encephalitis; CVA; cerebral mass; suspected dementia; epilepsy with cognitive impairment; genetic disorder; neurosurgery planned for epilepsy control; Parkinson's and other motor diseases; multiple sclerosis with cognitive impairment; hydrocephalus; mild cognitive impairment; other documented medical/neurological conditions.

Prior Authorization

Prior Authorization Required

Prior authorization is required for neuropsychological testing. The prior authorization request must specify the CPT codes and units requested (96132–96139) and include the specific tests or subtests to be administered when applicable. Total units and total time requested should be documented on the request form.

Required Supporting Documentation

Include relevant medical records and clinical documentation to support medical necessity. Provide dates and details for listed clinical indications (for example: traumatic brain injury, anoxic/hypoxic injury, intracranial surgery, neurotoxin exposure including cancer treatments, encephalitis, CVA, cerebral mass, suspected dementia, epilepsy with cognitive impairment, genetic disorders, planned neurosurgery for epilepsy, Parkinson's disease or other motor diseases, multiple sclerosis with cognitive impairment, hydrocephalus, mild cognitive impairment). Also include clinical assessment elements (clinical interview, neurologic exam, consultations, neuroimaging, EEG, psychiatric evaluation, neurobehavioral exam, rating scales, developmental/psychosocial history) and history of prior testing, medications, and substance use as applicable.

Background and Rationale

Neuropsychological testing is a comprehensive evaluation of cognitive, motor, and behavioral functional abilities related to developmental, degenerative, and acquired brain disorders. It is intended to support diagnosis and management when impairment is linked to a medical or neurologic condition and when results will affect medical treatment planning. Routine repeat testing is generally not warranted unless justified by supporting medical documentation; repeat testing may be authorized when clinically indicated.
