To be eligible the member must have an eligible chronic condition from the listed conditions AND demonstrate that the condition meets all three overarching requirements: is life‑threatening or significantly limits overall health or function, has a high risk of hospitalization or other adverse health outcomes, and requires intensive care coordination.

Demonstration of those requirements must include one or more specified utilization/function criteria (examples below) such as inpatient admissions, urgent care/ER visits, outpatient visit frequency, need for home health, impairment in activities of daily living, or need for outpatient durable medical equipment (DME).
