Coverage criteria for biologic materials

Documentation-based medical necessity criteria

Covered when ALL of the following are documented (when covered by specific group contract):

Documentation prerequisites: Pretreatment periapical radiographic images that are diagnostic quality, properly oriented, identified, and dated within 12 months.radiograph within 12 months

Required diagnostic-quality, properly oriented, identified pretreatment periapical radiographs dated within 12 months.

Periodontal charting: Current, dated 6-point periodontal charting indicating a minimum of 5 mm pocket depths.>=5 mm pocket depths

Documentation must include current dated 6-point periodontal charting showing at least 5 mm pocket depths.

Rationale for procedure/material: Rationale in support of the regeneration procedure and the type of biologic material being used may be requested (examples: PRP, Emdogain®, rhBMP, GEM 21S®).

Provide clinical rationale for procedure and selected biologic material.

The use of biologic materials for soft tissue or osseous regeneration may not be considered when performed in conjunction with other regenerative or surgical procedures. Examples include combination with soft tissue grafting, bone grafts, guided tissue regeneration, ridge augmentation, periradicular surgery, placement within extraction sites, or when used together with other regenerative materials. Coverage in these situations depends on the member’s specific group contract and may result in denial or non‑coverage.

When coverage is dependent on documentation (per specific group contract), failure to provide the required pretreatment radiographs, current 6‑point periodontal charting, and rationale may result in noncoverage or denial.

Document rationale for the regeneration procedure and selected biologic material (examples: platelet rich plasma, Emdogain®, recombinant human BMP, GEM 21S®).
Note that biologic materials may not be considered when used with soft tissue grafting, bone grafts, guided tissue regeneration, ridge augmentation, periradicular surgery, within extraction sites, or combined with other regenerative materials regardless of group plan coverage.

Clinical background

Regenerative dental therapies employ biologic materials to promote regeneration of periodontal and osseous tissues either as alternatives to or adjuncts with grafting procedures. These adjunctive products include examples such as platelet-rich plasma (PRP), Emdogain®, recombinant human bone morphogenetic protein (rhBMP), and GEM 21S®. When coverage is available under a specific group contract, indications must be documented with diagnostic pretreatment radiographs, current 6‑point periodontal charting showing the required pocket depths, and a rationale for the selected regenerative procedure and material.
