This policy governs utilization review and the determination of medical/dental necessity for mucogingival surgery and soft tissue grafting, defining required documentation, clinical indications, and limits for benefit determination. Coverage and benefit applicability are group contract dependent, and mucogingival surgery is considered for treatment of periodontal defects on natural teeth only.

Documentation Required Prior to Mucogingival Surgery

The following must be specifically documented prior to mucogingival surgery:

Attached gingiva: Current, dated periodontal chart documenting millimeters of attached gingiva
Pocket depth measurements: Current, dated periodontal chart documenting pocket depth measurements (six points/tooth)six points per tooth
Progressive recession history: History of progressive recession within 12 months prior to treatmentwithin 12 months
Frenum notation: Notation related to the presence of high frenum attachments
Number of teeth affected
Photographic documentation (conditional): Photographic documentation of areas demonstrating recession may be required

Medical Necessity Criteria for Mucogingival Surgery

Covered when ALL of the following are met:

Keratinized/attached gingiva threshold: Periodontal charting indicates recession noting 2 mm of keratinized gingival tissue and one millimeter or less of attached gingiva remainingKeratinized gingival tissue width = 2 mm; Attached gingiva remaining = <= 1 mm
Clinical indications present: Progressive gingival recession or loss of attached gingiva with concomitant root exposure, absence/reduced keratinized attached gingiva, pocket depth probing beyond mucogingival junction, high frenum attachments, or inadequate vestibular depth (clinical indications)

General Criteria / Limitations

Evaluation and post-op care included: Mucogingival surgical procedures include all evaluation and post-operative care for three months and any surgical re-entry for three years3 months post-op care; 3 years surgical re-entry
Natural teeth only: Mucogingival surgery will be considered for treatment of periodontal defects on natural teeth only
Frequency/benefit limits: Benefits are group contract dependent but generally limited to one (1) periodontal surgical procedure in a [36/60] month period per single tooth or multiple teeth in the same quadrantone procedure per [36/60] months per tooth/quadrant

Also document history of progressive recession within the prior 12 months, notation of high frenum attachments, number of teeth affected, and include photographic documentation and radiographs as required. Record indication of root sensitivity and relationship to cervical caries/existing restorations. Include a statement that the procedure is not cosmetic.

"Generally accepted standards of dental practice" are defined as standards based on credible scientific evidence published in peer-reviewed dental literature, specialty society recommendations/criteria, the views of recognized dentists practicing in the relevant clinical area, and any other relevant factors from credible sources.
