Coverage Summary

Veneers are a conservative restorative option applied to anterior teeth that have sustained significant loss of tooth structure (for example, injury involving one or both incisal angles or a canine cusp tip) where the remaining tooth can support a labial restoration.

Coverage is mixed — benefits are allowed when placement is clinically necessary, meets generally accepted standards of care, and there is a reasonable expectation for success; coverage depends on clinical documentation, prognosis, and the member's contract language. Policy number 02-902 with an effective (publish) date of 01/01/2025 and last review 10/25/2024 (minor editorial refinements noted).

Medically Appropriate Placement of Veneers

Covered when ALL of the following are met:

Placement of veneers must be necessary, appropriate and meet generally accepted standards of care.

Must have a reasonable expectation for success.

A current (within 12 months), pretreatment periapical radiographic image showing the apex is required for evaluation of lack of pulpal and/or periapical pathosis.Radiograph recency: within 12 months

Radiograph must be within 12 months prior to treatment.

Must have fully intact lingual surfaces on anterior teeth that can support veneer placement when there are large areas of decay.

If areas of decay are too large, a full crown may be more appropriate.

The periodontal health and long-term prognosis of the teeth must be considered.

Documentation relative to the history of definitive periodontal treatment is required; veneers are not appropriate with uncontrolled or untreated periodontal disease.

To be considered for a labial veneer, ALL of the following must be met:

Tooth must demonstrate significant loss of tooth structure and/or involvement of one or both incisal angles or cusp tip of a canine.
Upon request the degree of tooth breakdown must be documented by report, clinical chart notes, and/or scanned or photographic image.

Benefits for replacement of defective veneers (e.g. veneers that have become defective after initial placement because of normal wear, fracture or caries) is based on contractual frequency limitations.

Replacement coverage determined by member's contract frequency limits.

Veneers are NOT considered appropriate for the following (any one applies -> not covered):

For cosmetic purposes.

For correction of congenital/developmental problems.

For correction of tooth size discrepancies.

Restorative procedures for repair of complications from wear, attrition, abrasion, erosion, and abfraction do not meet contractual criteria for treatment.

Patients who demonstrate severe bruxism or grinding.

Veneers are not appropriate restorations for severe bruxism or grinding due to compromised long-term prognosis.

Coverage for replacement of defective veneers (for example, those that become defective after initial placement because of normal wear, fracture, or caries) is determined by the member's contract frequency limitations; verify the member's contract benefit frequency prior to authorization or billing.
