Defect characteristics: Defect is generally confined to vertical, multi‑walled or narrow defects with areas of vertical bone loss or a Class II furcation defect.

GTR should generally be confined to these defect types.

Treatment setting and limitations: Procedure is intended for periodontal or peri‑implant defects (coverage dependent on group contract); not considered when performed in conjunction with soft tissue grafting; procedures associated with endodontic therapies or minor periradicular surgery are typically not covered.

Refer to group contract for peri‑implant and implant‑related coverage.

Documentation requirements: Diagnostic radiographs that are recent, properly oriented and identified demonstrating vertical bone defects AND current (within 12 months) dated 6‑point periodontal charting showing pocket depths of at least 5 mm.Radiographs; charting within 12 months; pocket depth >= 5 mm

Both radiographic and clinical periodontal documentation are required for medical/dental necessity review.

Biologic materials restriction: Use of biologic materials for soft or osseous tissue regeneration will not be considered in conjunction with guided tissue regeneration.

Biologics are excluded from concurrent consideration.

Postoperative management: Post‑operative management included for the immediate three months following surgery; any surgical re‑entry management may be considered for up to three years (coverage dependent on group contract).3 months; 3 years

Guided tissue regeneration is a surgical periodontal technique intended to promote the reformation of new bone, cementum, and periodontal ligament attachment around a periodontally diseased tooth.
