Reimbursable Thyroid Testing and Monitoring

Primary reimbursement criteria

Reimbursable thyroid testing and monitoring when medically indicated per listed clinical scenarios:

ALL of the following

Primary hypothyroidism
- TSH to confirm or rule out primary hypothyroidism for individuals with signs and symptoms consistent with hypothyroidism (See Note 1).
- Free T4 (fT4) testing as a follow-up to abnormal TSH findings.
- For individuals treated for primary hypothyroidism: monitor with TSH and fT4 every 6 weeks upon dose change and annually when stable.

ALL of the following

Secondary/central hypothyroidism
- TSH and fT4 testing in cases of suspected secondary hypothyroidism.
- For individuals treated for secondary hypothyroidism: monitor with fT4 every 6 weeks upon dose change and annually when stable.

ALL of the following

Hyperthyroidism
- TSH testing to confirm or rule out overt hyperthyroidism.
- Total T3 (TT3) or free T3 (fT3) to confirm diagnosis of hyperthyroidism.
- Free T4 testing as a follow-up to abnormal TSH and to distinguish overt vs subclinical hyperthyroidism.
- For individuals treated for hyperthyroidism: repeat testing of TSH and fT4 every 8 weeks; annual monitoring after the first year even if asymptomatic for relapse or late-onset hypothyroidism.

ALL of the following

Pregnancy and postpartum
- For pregnant or postpartum individuals with symptoms of thyroid dysfunction: TSH and fT4 testing once every 4 weeks; testing should be performed at laboratories with trimester-specific reference ranges.
- For pregnant or postpartum individuals diagnosed with hyperthyroidism: TT4, TgAb, TRAb, and TPOAb may be reimbursable.

ALL of the following

Screening and high-risk asymptomatic individuals
- One-time TSH screening may be reimbursable for asymptomatic individuals at high risk (personal/family history of thyroid dysfunction; type 1 diabetes or other autoimmune disease; disease or neoplasm of the thyroid or other endocrine glands; chronic or acute urticaria; pediatric short stature or failure-to-thrive).
- TSH testing for individuals capable of becoming pregnant who have experienced two or more pregnancy losses.
- TSH testing for individuals with a thyroid nodule.

ALL of the following

Medication and therapy-related monitoring
- For asymptomatic individuals prescribed drugs that can interfere with thyroid function: TSH annually, when dosage/medication changes, and if symptoms develop.
- For individuals undergoing immune reconstitution therapy (IRT) including HIV on HAART, post-allogeneic BMT/HSCT, and patients on alemtuzumab: TSH testing once every 3 months with reflex fT4 and fT3 when abnormal.

ALL of the following

Special populations and monitoring intervals
- For hypothalamic-pituitary disease: monitoring of TSH and fT4 biannually for individuals <18 years and annually for individuals ≥18 years.
- Annual screening of TSH and fT4 for individuals diagnosed with primary mitochondrial disease (pediatric and adult intervals specified).

ALL of the following

Antibodies and cancer surveillance
- For individuals with hypothyroidism or hyperthyroidism: testing once every three years for thyroid antibodies (Tg-Ab, TPO-Ab, TRAb, TSI) may be reimbursable.
- For individuals with thyroid cancer: testing for serum thyroglobulin and/or Tg-Ab for detection of tumor recurrence, post-surgical evaluation, surveillance, and maintenance for differentiated thyroid carcinomas may be reimbursable.

ALL of the following

Non-reimbursable tests and general exclusions
- Testing for TRH or thyroxine-binding globulin (TBG) to evaluate cause of thyroid dysfunction is not reimbursable.
- Reverse T3 uptake and total T4 testing are not reimbursable for situations not mentioned above.
- Measurement of total T3 and/or free T3 for assessment of hypothyroidism is not reimbursable.
- Thyroid dysfunction testing during a general exam in asymptomatic nonpregnant individuals without abnormal findings is not reimbursable.

Providers must submit accurate documentation of services performed and bill using valid HIPAA-approved code sets. Claims are subject to plan documents, provider contract terms, medical policies, clinical payment and coding policies, code edit protocols, and coding software logic; providers may be asked to submit additional documentation upon request.

Submit claims using valid code combinations from HIPAA-approved code sets (CPT, HCPCS, ICD-10, NDC, DRG guidelines).
Ensure coding follows industry-standard coding guidelines (Uniform Billing Editor, AMA CPT resources, CMS NCCI/CCI rules).
Be prepared to provide additional documentation when requested for claim review or audits.
