Sepsis Coverage Criteria

Sepsis coding and documentation criteria

Claims are reviewed for documentation of infection, organ dysfunction from a dysregulated host response, and treatment consistent with the infection; the plan may request supporting documentation.

Claims must contain coding that accurately describes the services rendered and be supported by medical record documentation.

Documentation must show evidence of infection; for conditions such as viremia or fungemia, the evidence of infection must be noted in the provider statement (negative or inconclusive blood cultures do not preclude coding if provider documents infection evidence).

Claims must include evidence of end-organ dysfunction triggered by a dysregulated host response to infection when coding sepsis; organ dysfunction is a component of the sepsis definition and should not be coded separately from sepsis.

Treatment documented must be consistent with the type of infection present; the plan will review treatment course as part of the claims review and may request supporting documentation.
