Coverage Criteria for Therapy, DME, and Related Services

Therapy coverage criteria

Conditions that must be documented for coverage of physical medicine and rehabilitation CPT codes:

ALL of the following

The therapy must be of a skilled nature and require the services of a qualified healthcare professional.
When services are delegated, documentation must include details of what was delegated and who performed the service(s).
The services/therapy must not be maintenance in nature (ongoing treatment after a condition has stabilized or reached maximum medical improvement is considered maintenance care and is not eligible).
Services performed must achieve a specific diagnosis-related goal.
There must be a documented expectation that the member will achieve reasonable improvement over a predictable period of time for the services to be eligible for reimbursement.
Physical therapy for performance of athletic conditioning is not reimbursable by the Plan.

E/M services limitation

Limitations on reimbursement:

To bill for an E/M service, the complete CPT guidelines must be met for each service and the service must be separately identifiable and distinct from any other service performed on the same day.
The Plan will not reimburse Physical and Occupational Therapists or Physical and Occupational Therapy Assistants for CPT evaluation and management codes 99202-99499.

Timed code reporting

Rules for reporting units of timed therapy codes:

Use the 8-minute midpoint rule: a unit of time is attained when the midpoint is passed (for 15-minute units, report a single unit for treatment ≥ 8 minutes through 22 minutes).
Time reported must be the time actually spent in delivery of the modality or therapeutic procedure; pre- and post-delivery services and waiting/resting time are excluded.
All treatment time, including start and end times of direct treatment, must be recorded in the member's medical record with a note describing the specific modality or procedure.
Each minute of time may only be counted once; time spent attending one-on-one to a supervised modality cannot be counted for any other service.

DME, components, and customized devices

DME and customized device billing requirements:

A prescription/written order must be readily available for DME rentals or purchase; it must be signed by the member's treating QHP and renewed for refills at least annually as appropriate.
The prescription/written order must include: member name and DOB; diagnosis; description of equipment/supplies ordered; ordering provider's rationale; date of order; expected duration of use (and frequency/refill frequency if appropriate); quantity if applicable; ordering provider's name, address, phone; and ordering provider's legible signature and date.
Repair, adjustment, or replacement of components and accessories of DME, and supplies necessary for effective functioning, should be billed with the code that best describes the DME item (e.g., use HCPCS A4595 for TENS replacement products rather than billing electrodes, leads, paste, or batteries separately).
Customized DME or prosthetic/orthotic devices specifically constructed to meet a member's need require an invoice with billing when no procedure or HCPCS code exists.

Repair, adjustment, or replacement of DME components, accessories, and necessary supplies must be billed with the code that best describes the DME item rather than billing component codes separately.
