Observation reimbursement criteria

Observation services are reimbursable only under the conditions and documentation requirements below.

Observation services reimbursable only when provided under a physician's order or the order of an authorized practitioner and are member-specific (not part of routine facility protocol).

Observation services appropriate when the member requires short-term evaluation, treatment, or monitoring as an alternative to inpatient hospitalization and does not meet inpatient level of care.

Observation time must be documented with start and end clock times in the medical record and reported as units (hours) on the claim form.

ALL of the following

Exclusions (any of the following make observation ineligible)
- Services not appropriate for observation including, but not limited to: chemotherapy administration; provision of a medical exam when no skilled services are required; routine pre/post‑op recovery; services provided for convenience; planned overnight stays prior to testing when observation criteria not met; services that would normally require inpatient stay; services beyond routine treatment or procedure; services concurrently provided with procedures where active monitoring is part of the procedure; services provided when an inpatient is discharged to observation; services that are not reasonable and necessary; observation status does not apply when a member is treated as an outpatient only for administration of blood and receives no other medical treatment.
- Observation services beyond 48 hours are generally not appropriate (some contracts may preclude this); observation services beyond 72 hours are not eligible for reimbursement.

ALL of the following

Hospitals must bill HCPCS G0378 for hourly observation services with revenue code 0762; units reported must equal the number of hours the member is in observation status. Hospitals will not be allowed to bill more than one line of revenue code 0762 for observation status on the UB-04 claim form.
HCPCS G0379 is used for direct community referrals to observation (no associated ER/outpatient/critical care visit on the same date); G0379 may be reported with only one unit and must be billed in conjunction with G0378.

When observation care and an inpatient admission occur on the same date, observation services are handled per billing guidance (observation included on the appropriate claim type and revenue code mapping); if observation is followed by inpatient admission, observation services are included on the inpatient claim and not billed separately.

ALL of the following

Required supporting documentation includes the attending physician's order for observation care with clock time (or a nurse's observation admission note with clock time), physician admission and progress notes confirming need for observation, supporting diagnostic/ancillary reports, admission progress notes with clock time, discharge notes with clock time and discharge order, and signed physician/qualified nonphysician practitioner documentation during observation care.

Observation does not require prior authorization. However, if the member converts from observation to inpatient, the admission may require prior authorization.
Claims are subject to plan review; the plan may request supporting documentation and apply code edit protocols and review against plan documents and provider contracts.

Professional providers must follow guidance for E/M reporting when observation care and inpatient admission occur (see CPT 99221–99239 and related rules); when a member is admitted as inpatient, the performing provider must not separately report additional observation E/M services inconsistent with inpatient reporting rules.
