Covered when ALL of the following are met:

Serum ferritin reimbursable when ONE of the following applies

For the evaluation of an individual with abnormal hemoglobin and/or hematocrit levels.
For the evaluation and monitoring of iron overload disorders.
For individuals with symptoms of hemochromatosis (see Note 1).
For individuals with first-degree relatives with confirmed hereditary hemochromatosis (HH).
For the evaluation of individuals with liver disease.
For the evaluation of hemophagocytic lymphohistiocytosis (HLH) and Still Disease.
In males with secondary hypogonadism.
Monitoring at a frequency of every 1 to 3 months for: (1) patients with chronic kidney disease who are receiving or being considered for treatment for anemia; and (2) individuals on iron therapy.

Serum transferrin saturation reimbursable when ONE of the following applies

For the evaluation of iron overload in individuals with symptoms of hemochromatosis (see Note 1).
For the evaluation of iron overload in individuals with first-degree relatives with confirmed hereditary hemochromatosis (HH).
For the evaluation of iron deficiency anemia.

Explicit non-reimbursable tests

Serum hepcidin testing, including immunoassays, is not reimbursable.
Use of GlycA testing to measure or monitor transferrin or other glycosylated proteins is not reimbursable.

Other restrictions

Carbohydrate‑deficient transferrin is out of scope for this policy (not addressed here).
Any measurement of ferritin or transferrin levels, including transferrin saturation, performed for situations not explicitly listed as reimbursable above is not reimbursable.

Notes

Note 1: Symptoms of hemochromatosis include joint pain; fatigue; unexplained weight loss; abnormal bronze or gray skin color; abdominal pain; loss of sex drive.
Note 2: First‑degree relatives include parents, full siblings, and children of the individual.

Providers are responsible for submitting accurate documentation and claims using valid HIPAA-approved code sets. Claims must be coded per industry-standard guidelines (including UB editor, AMA CPT, CPT Assistant, HCPCS, ICD-10-CM/PCS, NDCs, DRG rules, CMS NCCI policy manual and edits). Claims are subject to code-edit protocols and review against benefit coverage, provider contract terms, medical policies, clinical payment and coding policies, and coding software logic.
