Serum ferritin reimbursable when ONE of the following applies
- Evaluation and monitoring of iron overload disorders (including evaluation of an individual with abnormal hemoglobin and/or hematocrit).
- Individuals with symptoms of hemochromatosis (See Note 1).
- Evaluation of individuals with liver disease.
- First-degree relatives of individuals with confirmed hereditary hemochromatosis (HH).
- Evaluation of hemophagocytic lymphohistiocytosis (HLH) and Still disease.
- Males with secondary hypogonadism.
- Evaluation and monitoring of patients with chronic kidney disease who are receiving or being considered for treatment for anemia; monitoring frequency typically every 1 to 3 months while on or being considered for therapy.
Transferrin saturation reimbursable when ONE of the following applies
- Evaluation of iron overload in individuals with symptoms of hemochromatosis (See Note 1).
- Evaluation of iron overload in first-degree relatives of individuals with confirmed hereditary hemochromatosis (HH).
- Evaluation of iron deficiency anemia.
ALL of the following
- Serum hepcidin testing, including immunoassays, is not reimbursable.
- Use of GlycA testing to measure or monitor transferrin or other glycosylated proteins is not reimbursable (carbohydrate-deficient transferrin is out of scope).
