Coverage and Billing Criteria for IONM

INV-01: IONM coverage and billing criteria

Covered when ALL of the following are met:

IONM must be provided to the operating surgeon by a licensed physician separate from the surgical team (operating surgeon, assistant surgeons, and/or anesthesiologists).
Interpreting physician presence (ONE of)
- Interpreting physician is physically in attendance in the operating suite.
- Interpreting physician is present by means of a real-time remote mechanism with live video (screen sharing identical to the information seen by the technician) and a high-quality bi-directional live audio connection, is constantly available to interpret the recording and advise the surgeon, and can converse with the operating surgeon at any time.
The monitoring professional must be solely dedicated to performing the intraoperative neurophysiologic monitoring and be available to intervene at all times during the reported monitoring time period(s).
Recording and testing are performed personally or by a technologist physically present with the member; the time reported reflects monitoring time only (excludes time to set up, record and interpret the baseline study, and to remove electrodes).
Services of the in-room technician and monitoring equipment are not separately payable from reimbursement to the monitoring physician; reimbursement for technician services and equipment may be negotiated with the facility.
Physicians performing the operative or anesthesia procedure may not bill IONM separately because it is included in the global surgical package.
Baseline studies should not be billed more than once per operative session; additional timed codes (e.g., 95961) should not be billed inappropriately alongside IONM timed codes (a non-timed alternative such as 95822 may be more appropriate).
Units must reflect cumulative one-on-one monitoring time even if non-contiguous; unit attainment uses mid-point rules and minimum billable increments (G0453 and 95940 not billed for 7 minutes or less; 95941 not billed for 30 minutes or less).
Modifier -26 must be appended for the professional component of monitoring and test interpretation where applicable; Modifiers TC and 26 do not apply to CPT add-on code 95941 and HCPCS code G0453 as noted.
Place of service should reflect the patient location (e.g., POS 19, 21, 22) and IONM services should be billed to the BCBS plan where the IONM provider is physically located at the time of service.
Documentation must support separately reimbursable IONM and should include event/chat logs, audio/video connection details, start/stop times, communications, technologist name/credentials, and evidence of the interpreting physician's availability or presence as applicable; recordings need not be submitted but records must document what occurred.

When billing CPT add-on codes: report +95940 for each 15-minute increment of one-on-one in-room monitoring (not for remote monitoring; not billable for 7 minutes or less); report +95941 for each hour of remote/nearby monitoring for one or more overlapping sessions (not billable for 30 minutes or less).

When billing HCPCS code G0453: bill only for undivided attention to a single member in 15-minute units (not billable for 7 minutes or less); units reflect cumulative one-on-one monitoring time even if not continuous.

Minimum billable time for G0453G0453 may not be billed for 7 minutes or less; units are billed in 15-minute increments with unit attainment using the mid-point rule (example: 37 minutes = 2 units; 38 minutes required for 3 units).

Minimum billable time for CPT 9594095940 may not be billed for 7 minutes or less; report per each 15-minute interval when monitoring is performed in-person one-on-one in the operating room; units reflect cumulative one-on-one monitoring time even if non-contiguous.

Minimum billable time for CPT 9594195941 may not be billed for 30 minutes or less; report per hour for remote/nearby monitoring across one or more overlapping operative sessions; unit attainment uses mid-point rule (example: 2 hours 30 minutes = 2 units; 2 hours 31 minutes required for 3 units).

The plan reserves the right to request supporting documentation. Failure to adhere to coding and billing policies may impact claims processing and reimbursement. Providers should retain and be prepared to submit documentation when requested to substantiate services and reimbursement.

Event/chat log or contemporaneous documentation of monitoring
Copies of any requested audio/video screen-sharing session logs or evidence of live connection
Signed statement or note that the interpreting physician was separate from the surgical team
Start and stop times for services provided
Records of communications between monitoring physician and operating surgeon (baseline, intraoperative events, closing review)

Billing Rule

Separately Reimbursable Service Conditions

Services for intraoperative neurophysiologic monitoring (IONM) are reimbursable separately only when all provider and delivery conditions are met. Billing for technician services and monitoring equipment is not separately payable when included in the monitoring physician/company reimbursement; such arrangements may be negotiated with the facility.

Must be provided to the operating surgeon (ordering physician) by a licensed physician who is separate from the surgical team (operating surgeon, assistant surgeons, and/or anesthesiologists).
Interpreting physician must be physically in attendance in the operating suite OR present via real-time remote mechanism for all END/IONM monitoring situations.
If remote, the interpreting physician must be constantly available to interpret the recording and advise the surgeon throughout the monitoring period.
Remote presence requires live video (screen sharing) that duplicates the technician’s monitoring screen and a high-quality bi-directional live audio connection enabling the remote physician to converse with the operating surgeon at any time. Cell phone or cellular walkie‑talkie connections are not adequate.
Review of recordings or documentation postoperatively (for example, review of a CD) does not constitute live monitoring and is not separately reimbursable.
Technician services and monitoring equipment are not separately payable from the reimbursement to the monitoring physician or monitoring company; reimbursement for technician/equipment may be negotiated directly with the facility.
When the interpreting physician is in a different state than the member, submit the claim to the state plan where the provider (interpreting physician) is physically located at the time the service is rendered.
