Reimbursement and billing rules

Services that are mutually exclusive, integral to, incidental, or within the global period of a primary service are not eligible for separate reimbursement.

Routine services, equipment and supplies commonly furnished during injection/infusion (for example: local anesthesia, IV start, access to indwelling IV/subcutaneous catheter or port, flush at conclusion, standard tubing, syringes) are included in the general charge and are not separately reimbursed.

Some HCPCS supply codes are not separately reimbursable when the cost of the supplies is incorporated into an E/M service or procedure performed in a non‑facility place of service.

Hydration codes (96360-96361) are for IV hydration (fluids/electrolytes) and require a minimum service duration of 31 minutes; they are not to be used for IV to keep a line open or for incidental fluids during drug infusions.

Only one initial infusion/injection service code may be reported per member encounter per day for physician/QHP and facility reporting, unless protocol or the member's condition requires two separate IV sites; additional access may be reported by appending an appropriate modifier.

If the member returns for a separate visit/encounter on the same day and requires an infusion or injection, another initial infusion or injection code may be billed with modifier -59 for that service.

Sequential infusions (a different drug or service administered after completion of the first infusion through the same IV site) may be billed when documentation supports that the administration was sequential rather than concurrent; sequential infusions may be billed once per sequential infusion of the same fluid mix.

Administration time for an infusion/injection is a separate service from E/M; time spent on the injection/infusion should not be included when selecting the E/M level of service. Use modifier -25 when a significant, separately identifiable E/M is performed on the same day and documentation supports it.

Chemotherapy and other highly complex biologic administrations are reported using chemotherapy/complex administration codes (96401-96549 range referenced) and follow facility hierarchy for primary service determination.

Providers are responsible for coding claims according to industry standards; claims are subject to code edits, plan terms, provider contracts, and clinical payment and coding policies. The Plan may request supporting documentation and providers should submit documentation upon claim review to determine eligible reimbursement (including orders, drug name/dosage, start/stop times, rates, and signatures).

Administration and coding rules

Billing/coding criteria and examples for administered drugs and member-supplied drugs:

Report the administered drug using the HCPCS/CPT code(s) that accurately describe the drug and dosage; if no specific code exists, report an appropriate unlisted code with a description and supporting documentation.

Example: report HCPCS J2357 with CPT 96372 for a therapeutic, prophylactic, or diagnostic subcutaneous or intramuscular injection; append modifier JW for discarded/not administered amount or JZ if there was zero drug amount discarded/not administered.

If a member supplies their own drug and no E/M or other EIM service is provided, report the appropriate CPT administration code (for example, 96372) and include a separate line for the drug and dosage with a zero-dollar ($0.00) charge.

When reporting discarded biologic/drug amounts in non‑inpatient settings, use modifiers JW and JZ on separate lines and document dosage and reason for discard.

Providers should select codes that accurately describe the administered services and are responsible for submitting supporting documentation when requested to substantiate billing and reimbursement.

If no existing code adequately describes the service, report an appropriate unlisted code with a description and supporting documentation. For administration of patient‑supplied drugs or when infusion/administration services are not provided, report the appropriate CPT administration code and, if required, include a separate line for the drug with a $0.00 charge.
