General Inflammation Testing (CRP and ESR) Reimbursement
This policy describes reimbursement criteria for measurement of C-reactive protein (CRP) and erythrocyte sedimentation rate (ESR) for various inflammatory conditions and the conditions under which testing is not reimbursable; it applies to providers submitting claims to Blue Cross Blue Shield of Texas.
Added Note 1 referencing CPCPLAB046 for the use of CRP as a cardiac biomarker, and CPCPLAB047 for the use of CRP as a marker for acute pancreatitis.
Removed 'inflammatory' from #1 as some conditions listed in Table 1 are noninflammatory, with measurement of CRP/ESR used to differentiate between a noninflammatory and inflammatory condition.
Revised Table 1 introduction to state coverage is designated based on the diagnosed or suspected inflammatory condition and that either conventional or high-sensitivity CRP testing are allowed; when either CRP or ESR are allowed, CRP is preferred and if both ordered only CRP will be allowed.
Within Table 1, added '(e.g., inflammatory bowel disease)' in the Frequency of Testing column for Irritable Bowel Syndrome and deleted the row specific to Giant Cell Arteritis; added Giant Cell Arteritis and Takayasu Arteritis under Large Vessel Vasculitis and changed Test Preference to CRP and ESR with specified monitoring frequency.
Added CPT/HCPCS code 86141 to the policy coding list.
