Coverage determinations for specific testing scenarios:

ALL of the following

ANA screening
- ALL of the following
  - Once during initial workup
  - Up to two additional tests per lifetime if new or more severe signs or symptoms of an autoimmune disease develop
ENA panel testing
- Individuals with an abnormal, raised ANA titer and clinical correlation with the appropriate autoimmune disorder: ENA panel testing of specific autoantibodies may be reimbursable
RF and/or anti-CCP testing
- Individuals with painful and swollen joints suggestive of rheumatoid arthritis (RA): testing for rheumatoid factor (RF) and/or anti-cyclic citrullinated peptide (anti-CCP) antibodies
- Once during initial workup
- If initial testing did not result in a diagnosis of RA, up to two additional tests per lifetime if symptoms persist or additional symptoms of RA develop
dsDNA testing
- Individuals with an initial positive ANA test and a diagnosis of systemic autoimmune rheumatic disease: dsDNA testing up to four times per year may be reimbursable

Documentation and claims review

Providers are responsible for submission of accurate documentation of services performed. Providers must submit claims using valid HIPAA-approved code sets and code appropriately per industry standards (e.g., UB, AMA CPT, CPT Assistant, HCPCS, ICD‑10, NDCs, DRG, CMS NCCI). Claims are subject to code edit protocols and review against benefit coverage, provider contracts, medical and coding policies, and coding software logic. Upon request, providers should submit any additional documentation to support claims.

Submit claims using valid HIPAA-approved code sets and industry coding guidelines.
Be prepared to provide supporting documentation upon request for claim review.
