Vitamin B12 and Methylmalonic Acid Testing
This policy governs reimbursement criteria for serum vitamin B12 (cobalamin) and methylmalonic acid testing for Blue Cross Blue Shield of Texas members and applies to providers submitting claims under BCBSTX plan documents.
A three-month testing frequency limit was added: total vitamin B12 (serum cobalamin) testing may be reimbursable once every three months for specified situations.
Holotranscobalamin testing is not reimbursable for screening, testing, or confirmation of vitamin B12 deficiency.
A catch-all noncoverage statement: for all other situations not described above, total vitamin B12 (serum cobalamin) testing is not reimbursable.
