ALL of the following

For individuals presenting with signs and symptoms of acute pancreatitis (see NOTE 1), measurement of either serum lipase (preferred) or amylase concentrations may be reimbursable.
- Not reimbursable when any of the following apply
  - The individual has an established diagnosis of acute or chronic pancreatitis.
  - Testing is performed more than once per visit.
  - Testing is performed for asymptomatic individuals during a general exam without abnormal findings.
For the diagnosis, assessment, prognosis, and/or determination of severity of acute pancreatitis, measurement of serum and urine trypsin/trypsinogen/TAP (trypsinogen activation peptide) is not reimbursable.
- Not reimbursable biomarkers
  - C-Reactive Protein (CRP)
  - Interleukin-6 (IL-6)
  - Interleukin-8 (IL-8)
  - Procalcitonin
For individuals presenting with signs and symptoms of acute pancreatitis (see NOTE 1), measurement of urinary amylase concentration for the initial diagnosis of acute pancreatitis is not reimbursable.
For all other situations or conditions not described above, measurement of serum lipase and/or amylase is not reimbursable.

Documentation Required

Documentation and claim review — Providers must submit accurate documentation

Providers are responsible for submission of accurate documentation of services performed. Providers must submit claims using valid code combinations from HIPAA-approved code sets and code services appropriately per industry-standard coding guidelines (e.g., Uniform Billing, AMA CPT, CPT Assistant, HCPCS, ICD-10-CM/PCS, NDCs, DRG guidance, CMS NCCI policy manual and edits). Claims are subject to code edit protocols and review against benefit terms, provider contract language, medical policies, clinical payment and coding policies, and coding software logic. Upon request, providers should furnish additional documentation to support medical necessity and coding. Claims may be denied, down-coded, or recouped if documentation is incomplete, inconsistent with billed codes, or does not support the service rendered.

Respond to requests for additional documentation promptly to avoid claim denials or delays.
