Adult noninvasive diagnostic indications: For individuals >=18 years, urea breath testing OR stool antigen testing may be reimbursable when ANY of the following are present: (a) dyspepsia (see Note 1), (b) active peptic ulcer disease, (c) past peptic ulcer disease with recurrent symptoms, (d) low-grade gastric mucosa-associated lymphoid tissue (MALT) lymphoma, (e) history of resection of early gastric cancer, (f) gastric intestinal metaplasia, (g) initiating chronic or on long-term aspirin or non-steroidal anti-inflammatory therapy, (h) unexplained iron deficiency anemia, (i) idiopathic thrombocytopenic purpura, (j) family history of gastric cancer, (k) first-generation immigrant from a high-prevalence area.adults >=18

Urea breath testing and stool antigen testing are not reimbursable when used to diagnose H. pylori infection in asymptomatic individuals of all ages. In addition, these noninvasive tests are not reimbursable for adults (>=18 years) who have only typical gastroesophageal reflux disease (GERD) symptoms (heartburn, regurgitation) and no history of peptic ulcer disease.

Serologic testing for H. pylori infection is not reimbursable for individuals of all ages and should not be used to establish active infection for reimbursement purposes.

Billing Rule

Providers are responsible for submission of accurate documentation of services performed and should use valid HIPAA-approved code sets. Providers are expected to submit claims using appropriate code combinations and to follow industry standard coding guidelines. Claims are subject to code edit protocols and may be reviewed against benefit coverage, provider contract language, applicable policies, and coding software logic.

Providers must submit supporting documentation with claims to demonstrate medical necessity when requested. Documentation should include relevant clinical information such as patient age, presenting symptoms, endoscopic findings (if performed), prior treatment history (including dates and agents used), and results of prior H. pylori testing. For test-of-eradication claims, document that testing was performed at least four weeks after completion of therapy.

Common denial triggers include testing for asymptomatic individuals, serologic testing for H. pylori, testing adults with typical GERD symptoms without history of peptic ulcer disease, repeat or concurrent diagnostic testing (urea breath test, stool antigen, biopsy) for the same diagnostic episode, and testing performed while the patient recently used antibiotics, proton pump inhibitors, or bismuth. For pediatric patients, biopsy-based testing with rapid urease or culture for certain indications (functional abdominal pain, initial iron deficiency anemia workup, short stature evaluation) is not reimbursable.

Background

Helicobacter pylori infection may be identified using both noninvasive and invasive diagnostic methods. Common noninvasive methods include the urea breath test and stool antigen testing, which are used for initial diagnosis in selected clinical scenarios and to confirm eradication when performed at least 4 weeks post-treatment. Invasive, biopsy-based testing performed at endoscopy includes histology with a biopsy plus an additional test such as a rapid urease test or culture with susceptibility testing; biopsy-based testing is indicated in adults undergoing endoscopy or those with alarm features. Serologic testing is not recommended for detecting active infection. Nucleic acid testing is generally not reimbursable for diagnosis except when used for susceptibility testing in refractory infection.
