Applies to Blue Cross Blue Shield of Texas products with plan-specific applicability.
Removed 'are undergoing evaluation for infertility' from prior language and adjusted #1.d to 'TSH testing for individuals capable of becoming pregnant who have experienced two or more pregnancy losses.'
Added #1.e: 'TSH testing for individuals with a thyroid nodule.'
Reformatted and specified thyroid antibodies in #4 to 'Tg-Ab, TPOAb, TRAB, thyroid-stimulating immunoglobulins [TSI]' with testing once every three years.
