Routine urine culture testing for initial screening of asymptomatic prostatitis or as part of assessment/prognosis around prostate biopsy is not supported by the evidence and DOES NOT MEET COVERAGE CRITERIA. Studies cited in the source found similar complication rates with and without routine pre‑biopsy culture and low clinical utility for routine pre‑biopsy cultures (Bruyere et al., 2010), supporting exclusion of these indications from coverage.

Exceptions where screening and treatment of asymptomatic bacteriuria (ASB) are recommended include: pregnant individuals (screen and treat ASB) and patients who will undergo endoscopic urologic procedures associated with mucosal trauma (obtain urine culture and treat ASB prior to the procedure). These exceptions reflect guideline recommendations (IDSA, EAU and transplant/ procedural guidance) that identify pregnancy and certain urologic interventions as clinically justified indications for screening and treatment.

Routine screening and treatment of ASB is not recommended for most nonpregnant adults and many other groups. Guideline panels (IDSA, EAU and allied societies) recommend against screening or treating ASB in infants and children, healthy premenopausal and postmenopausal persons, community‑dwelling functionally impaired older adults, long‑term care residents, patients with diabetes, many transplant recipients beyond specified windows, short‑term catheterized patients (<30 days), and patients undergoing elective nonurologic surgery. These recommendations are based on harms from unnecessary antibiotic use and lack of demonstrated benefit.

Do not perform urine testing (urinalysis, urine culture, blood culture or C. difficile testing) in the absence of signs or symptoms of infection. Choosing Wisely / SHEA and AMDA guidance advise withholding cultures until clinical criteria are met because testing asymptomatic patients yields false positives (eg, colonization or contamination), can prompt unnecessary antibiotics, and may delay correct diagnosis of the true cause of clinical deterioration. Documented signs and symptoms that justify testing are listed in the policy (fever, dysuria, urgency, frequency, flank pain, pyuria, hematuria, etc.).

Follow‑up urine culture testing after an uncomplicated UTI in patients who demonstrate clinical resolution DOES NOT MEET COVERAGE CRITERIA and is discouraged. Per the policy and guideline summaries, routine surveillance or repeat cultures after clinical recovery are unnecessary; obtain a repeat culture only if symptoms persist, recur, or clinical concern remains. This approach reduces unnecessary laboratory testing and inappropriate antibiotic exposures.

Screening for or treating asymptomatic bacteriuria in most asymptomatic patients is considered not medically necessary. Except for the explicit exceptions (eg, pregnancy, pre‑procedural mucosal breach), routine urine culture for ASB lacks evidence of benefit and may cause harm through unnecessary antibiotic treatment, longer hospital stays, and adverse events. Coverage determinations reflect these guideline‑based conclusions.

Specific groups for whom screening or treatment of ASB is discouraged include: infants and children; healthy premenopausal nonpregnant and healthy postmenopausal persons; older community‑dwelling or long‑term care residents; patients with diabetes; renal and nonrenal transplant recipients (beyond specified post‑transplant windows); and patients with short‑term indwelling urethral catheters (<30 days). These populations are cited in IDSA/EAU guidance recommending against routine screening/treatment unless other clinical indications exist.

The US Preventive Services Task Force (USPSTF) recommends against routine screening for asymptomatic bacteriuria in adults outside of pregnancy; in general, testing or treating ASB without symptoms is not supported by USPSTF guidance and is therefore discouraged in coverage decisions.
