Covered when ALL of the following are met:

Pathological examination of tissue obtained from a prostate biopsy involving 12 core extended sampling (see Note 1) for initial diagnosis of prostate cancer as a follow up to abnormal PSA results, presence of a palpable nodule on digital rectal examination, or suspicious radiologic findings.

Note 1: One vial per sextant, with no more than two core samples per vial.

Medically Necessary / Follow-up Biopsy

Covered when ALL of the following are met:

Pathological examination of tissue from a follow-up prostate biopsy (excluding prostate saturation biopsy) when clinical suspicion of prostate cancer remains in an individual whose initial biopsy was negative.

Not Medically Necessary / Does Not Meet Coverage Criteria

These do NOT meet coverage criteria:

Pathological examination of tissue obtained from a prostate saturation biopsy for the diagnosis, staging, or management of prostate cancer.

Saturation biopsy involves extensive sampling obtaining up to ~24 cores and is not appropriate for initial biopsy; associated with higher complication rates.

Background, Evidence & Definitions

Prostate cancer is the most common cancer among individuals with a prostate and a leading cause of cancer death in older adults, with an estimated 288,300 new cases and 34,700 deaths in the US in 2023; about 11% of individuals with a prostate will be diagnosed during their lifetime (Siegel et al., Taplin & Smith). Findings on DRE or elevated PSA prompt biopsy, but many prostate cancers are indolent and screening/biopsy decisions should be individualized.

Multiple sampling strategies exist to improve detection. The historical sextant (six-core) approach may miss ~30% of clinically significant cancers and has largely been replaced by extended 12-core sampling, which lateralizes cores and has been shown to detect ~31% more cancers than six-core schemes. Saturation biopsy (up to 20–24+ cores) can increase detection on repeat biopsy (reported ~22–33%) but carries higher complication rates and is not appropriate for initial biopsy. Multiparametric MRI (mpMRI) with targeted biopsy improves detection of clinically significant cancers and many guidelines support using mpMRI and targeted biopsy (often combined with systematic sampling) particularly for initial and repeat biopsy decisions; routine saturation biopsy is not recommended for all patients given MRI-targeted approaches and increased harms.
