Background & Evidence

Oral and oropharyngeal cancers are common and carry substantial mortality; estimates cited include roughly ~58,450 diagnoses and ~12,230 deaths in 2024 in the U.S., with squamous cell carcinoma as the predominant histology. Human papillomavirus (HPV) is an important contributor to oropharyngeal cancer, but there is no approved screening test for oral or oropharyngeal HPV infection. Numerous adjunctive, noninvasive tests have been evaluated—salivary biomarkers (mRNA, miRNA, proteins, metabolites), brush cytology, vital staining, autofluorescence and other light-based detection systems, and chemiluminescence devices—but evidence is variable. Systematic reviews and meta-analyses report wide ranges of sensitivity and specificity across modalities, promising results for salivary mRNA/miRNA in some studies, and mixed or limited performance for devices like VELscope and ViziLite. Overall, lack of standardization, interobserver variability, and insufficient high-quality prospective validation limit the clinical adoption of many adjunctive tests.
