One of the clinical indications

Persistent diarrhea or diarrhea with signs or risk factors for severe disease (e.g., fever, bloody diarrhea, dysentery, dehydration, severe abdominal pain) AND multiplex PCR-based gastrointestinal pathogen panel testing of up to 11 GIPs
No more often than once every 7 days for up to 11-target GIP panel
Signs and symptoms of a respiratory tract infection (e.g., temperature ≥ 102°F, pronounced dyspnea, tachypnea, tachycardia) AND multiplex PCR-based respiratory pathogen panel testing of up to 5 respiratory pathogens
Temperature ≥ 102°F qualifies as sign/symptom for respiratory tract infection indication; Allowed no more often than once every 7 days.

Claims for multiplex panels that exceed allowed target counts (≥12 GIP targets or ≥6 respiratory targets) or that are used for excluded specimen types (CSF, blood, urine for UTI diagnosis, wound infection screening, or general microorganism screening) do not meet coverage criteria and may be denied.

Clinical and Administrative Background

Multiplex amplified probe and multiplex reverse-transcription techniques allow detection and identification of multiple pathogens from a single clinical sample using a single assay. These methods can improve turnaround time and overall diagnostic yield compared with conventional methods such as culture, microscopy, and immunoassay, which often have lower sensitivity and longer reporting times (see Palavecino, 2019).

The clinical utility of broad multiplex panels depends on the care setting and whether test results will meaningfully change patient management. Evidence and guidance sources cited in this policy (including professional society guidance and systematic evaluations of multiplex panels) support more limited use of broad panels—favoring targeted panels when results inform therapy, infection control, or other clinical decisions—rather than routine broad, untargeted screening. In other words, multiplex panels can be valuable when results alter management, but their routine use is not supported across all settings.
