Alkaline phosphatase (ALP) — Indications: Bone neoplasms: workup; during treatment; surveillance; Systemic light chain amyloidosis: initial diagnostic workup.

Alpha fetoprotein (AFP) — Indications: Hepatocellular carcinoma: screening; workup for confirmed HCC; surveillance; Intrahepatic cholangiocarcinoma: workup for isolated intrahepatic mass.every 3-6 months for 2 years, then every 6 months
Surveillance frequency specified for HCC as every 3-6 months for 2 years, then every 6 months.

Denial Risk

Claims for unlisted/unsupported indications may be denied

Testing for tumor markers or panels not addressed in the covered indications, or the use of proteomic pattern analysis for screening/detection, does not meet coverage criteria and may be denied. Claims for testing outside the policy-specified indications or for proteomic serum pattern screening are at risk for denial.

Denial risk applies to tests/panels not listed under covered indications and to proteomic pattern analysis for cancer screening.

Billing Rule

Permitted surveillance frequency

Quarterly measurement of designated serum tumor markers is permitted for follow-up, monitoring, and/or surveillance except where the policy specifies alternative frequencies. Follow the specific surveillance intervals provided in the policy for certain markers and conditions.

AFP (hepatocellular carcinoma): every 3–6 months for 2 years, then every 6 months.
CA 19-9 (resected ampullary cancer stage I–III and some pancreatic/small bowel indications): every 3–6 months for 2 years, then every 6–12 months for up to 5 years (as clinically indicated).
CEA (colorectal/rectal cancer): every 3–6 months for 2 years, then every 6 months for a total of 5 years.
Germ cell / malignant germ cell tumor surveillance: no more than every 2 months for the first 2 years, every 4 months in years 3–5, then annually after year 5.
Beta-hCG for gestational trophoblastic neoplasia during/post treatment: no more than weekly during treatment; follow-up no more than monthly for 12 months.
Serum free light chain (multiple myeloma surveillance): up to once per month.

Circulating tumor biomarkers detected in serum are used to help detect, diagnose, stage, and manage certain cancers; their amounts are typically elevated in individuals harboring a tumor. This policy is constructed from recommendations in the NCCN Biomarkers Compendium® (NCCN, 2023) and references multiple guideline and evidence sources (NCCN, ASCO, ATA, NANETS, AASLD, NCI, and others).

The policy notes that many specific tests are available as laboratory‑developed tests (LDTs). LDTs are regulated by CMS under the Clinical Laboratory Improvement Amendments (CLIA) as high‑complexity tests and are not FDA‑approved/cleared; however, FDA clearance or approval is not currently required for clinical use of LDTs.
