Scientific background and diagnostic limits: The diagnosis of Crohn's disease and ulcerative colitis depends on a combination of clinical presentation and endoscopic, radiologic, histologic, and laboratory investigations. Serologic markers (notably pANCA and ASCA and other antiglycan or microbial antibodies) may support diagnostic considerations or help differentiate CD from UC in select situations, but their accuracy is limited and they are not sufficient as standalone diagnostic tests. Panels that combine serologic, genetic, and inflammatory markers (for example, Prometheus® panels) have produced higher area under the curve in some studies but their added clinical utility for routine patient management remains uncertain. Genetic testing identifies many susceptibility loci but known loci explain only about 13% of disease liability, limiting the predictive value of genetic testing except when evaluating suspected monogenic causes in infants and very young children.
