Outpatient measurement of serum drug levels and/or anti‑drug antibodies for the biologics listed in this policy is excluded (does not meet coverage) when performed for any reason other than the specific inflammatory bowel disease indications described elsewhere in this policy. Examples of non‑covered uses include measurement for individuals without IBD (such as those with spondyloarthritis, rheumatoid arthritis, psoriatic arthritis, or psoriasis) and testing performed in outpatient settings for reasons not specified under the IBD coverage criteria.

The National Institute for Health and Clinical Excellence (NICE) concluded that enzyme‑linked immunosorbent assay (ELISA) kits for therapeutic monitoring of TNF‑alpha inhibitors show promise but that there is insufficient evidence to recommend routine adoption outside of research or data‑collection contexts. Routine use of ELISA‑based TDM for TNF inhibitors in Crohn's disease or rheumatoid arthritis is therefore not supported by NICE.

For vedolizumab and ustekinumab, current evidence is insufficient to define specific induction or maintenance target concentrations. The guidance indicates that available data do not support numeric therapeutic targets beyond confirming that drug is detectable.

Testing for drug concentrations and antidrug antibodies performed outside the covered IBD indications is generally considered not medically necessary and does not meet coverage.
