General Inflammation Testing (CRP and ESR) Coverage Criteria
Policy governing coverage and limitations for measurement of C-reactive protein (CRP) and erythrocyte sedimentation rate (ESR) for diagnosing and monitoring inflammatory conditions for Blue Cross Blue Shield of Tennessee members.
Removed the word 'inflammatory' from CC1 and clarified that CRP/ESR measurement meets coverage criteria for the conditions specified in Note 1.
If CRP and ESR are ordered at the same time for a condition where CRP or ESR are allowed, only CRP will be approved.
Within Note 1's table, added '(e.g., inflammatory bowel disease)' in the 'Frequency of Testing' column for IBS for clarity.
