This policy defines coverage for measurement of pancreatic enzyme biomarkers in the evaluation of suspected acute pancreatitis. Coverage is granted for a single measurement of either serum lipase (preferred) or serum amylase when an individual presents with signs and symptoms of acute pancreatitis (see Note 1). The policy uses the Atlanta/ guideline biochemical threshold of serum lipase or amylase ≥3× the upper limit of normal (ULN) as the diagnostic enzyme cutoff. Coverage is not provided (not medically necessary) for multiple situations including repeat or serial measurement during the same visit, testing in individuals with an established diagnosis of acute or chronic pancreatitis, and for asymptomatic screening in general exams; additionally, several other tests/biomarkers are explicitly listed as not meeting coverage criteria (for example, measurement of serum or urine trypsin/trypsinogen/TAP, urinary amylase for initial diagnosis, and inflammatory biomarkers such as CRP, IL-6, IL-8, and procalcitonin). Background & Evidence Summary

Acute pancreatitis (AP) is an inflammatory condition that may be transient or progress to severe disease; chronic pancreatitis (CP) is a distinct, usually progressive inflammatory disorder marked by pain and exocrine insufficiency. Diagnostic guidance follows the revised Atlanta classification: diagnosis requires at least two of three criteria — characteristic abdominal pain, biochemical evidence (serum amylase or lipase >3× ULN), or characteristic cross-sectional imaging findings. Serum lipase and amylase both rise within hours of pancreatic injury, but lipase is generally more sensitive and remains elevated longer (days to weeks), making it the preferred initial test; many studies and guideline statements recommend testing lipase rather than amylase and discourage routine co-ordering of both. Urine and peritoneal enzyme testing (including urinary amylase and peritoneal amylase) and urine trypsinogen/trypsinogen-2 dipstick or TAP assays have been evaluated but yield variable and inconsistent diagnostic performance across studies; evidence reviews note limited convergence of results and variable sensitivity/specificity, and some point-of-care urine trypsinogen tests report lower sensitivity that limits routine use. Other biomarkers (CRP, procalcitonin, IL-6, IL-8, LDH, serum cholinesterase and combinations) have been studied as adjuncts to predict severity — combined panels may show high AUC in select studies (for example, CRP+PCT+IL-6+LDH reported AUC 0.989), and individual markers (eg, CRP) can help assess severity when timed appropriately — but guidelines do not support use of single inflammatory markers alone to triage patients to ICU or replace clinical scoring/imaging.
