Cytologic analysis of epithelial cells obtained from nipple aspiration methods (including ductal lavage and suction techniques) is excluded from coverage as an assessment technique for breast cancer risk. The policy states that such testing "DOES NOT MEET COVERAGE CRITERIA" because there is insufficient published scientific literature confirming the tests are required and beneficial for diagnosis or treatment.
The available evidence demonstrates limitations in diagnostic accuracy and uncertain clinical utility for nipple discharge cytology and ductal lavage. A retrospective series reported a sensitivity of 17% and specificity of 66%, leading authors to conclude that nipple discharge cytology offers little complementary diagnostic value. Systematic and pooled analyses also show mixed results: meta-analytic review identified modest associations between abnormal cytology and future cancer risk but relied on heterogeneous studies (16 articles, >20,000 aspirations) and does not establish proven benefit for patient management. For these reasons the policy finds the tests lack sufficient diagnostic performance and demonstrated clinical benefit to support coverage.
