- CPT 92002 — Ophthalmological services: medical examination and evaluation with initiation of diagnostic and treatment program; intermediate, new patient (covered when performed as a medical exam, not as routine preventive service).
- CPT 92004 — Ophthalmological services: medical examination and evaluation with initiation of diagnostic and treatment program; comprehensive, new patient (covered when initiation of diagnostic/treatment program is present).

RuleBlueCHiP for Medicare: the first claim processed in a calendar year with codes 92002–92014, S0620 or S0621 will be considered a routine eye exam for benefit application; subsequent such claims are adjudicated as medical specialist visits.

Filing, adjudication, and billing guidance for providers

Billing Rule

Standalone vision plan claim filing

If a member has a standalone vision plan, BCBSRI providers who are also participating providers with that vision plan must follow the vision plan's contractual and claims-filing guidelines. File routine eye care claims (including routine refraction/exam services) to the member's standalone vision benefit plan rather than to BCBSRI.

File routine eye care claims to the member's standalone vision plan when applicable.
HCPCS S0620 (new patient) and S0621 (established patient) should be used to direct routine/refraction exams to the vision benefit.
If the visit was scheduled as a routine exam but during the visit a medical diagnosis/treatment is initiated and the member lacks routine exam benefits, report the appropriate CPT code and follow the member's medical benefit.

Billing Rule

Adjudication of first vs subsequent ophthalmology claims (calendar year)

For BlueCHiP for Medicare, BCBSRI applies a calendar-year first-claim rule for eye exams. The first ophthalmology/optometry claim processed in a calendar year with the listed codes will be adjudicated as a routine eye exam for benefit application; subsequent claims are treated as medical specialist visits.

Effective 1/1/2019 for BlueCHiP for Medicare: The first claim processed by BCBSRI during a calendar year from an ophthalmologist or optometrist reported with codes 92002–92014, S0620 or S0621 is considered a routine eye exam regardless of diagnosis.
Routine eye exam benefits are generally limited to one exam per year; check member-specific benefits.
Any subsequent claims during the same calendar year billed with codes 92002–92014 will be considered a medical specialist visit for benefit application.
All E/M codes in the 99000 series (e.g., 99213) reported during the calendar year by an ophthalmologist or optometrist will be considered a medical specialist visit.

For member-specific benefits and eligibility questions, contact the BCBSRI provider call center. Providers may not charge members for services determined not medically necessary or for non-covered benefits unless the member was informed and agreed in writing in advance.

DefinitionIntermediate ophthalmologic exam: CPT 92002 (new) and 92012 (established) — evaluation of a new or existing condition complicated by a new diagnostic or management problem with initiation of a diagnostic and treatment program; includes history, general medical observation, external ocular and adnexal exam and other diagnostics as indicated.

When to useReport for patients needing care for a condition other than refractive error or routine screening; requires initiation or continuation of a diagnostic or treatment plan.

Not appropriate forScreening/preventive exams, prescription of lenses, or routine contact lens monitoring (unless care involves bandage lenses or keratoconus therapy).

Comprehensive ophthalmologic exam — Comprehensive services (92004, 92014)

DefinitionComprehensive ophthalmologic exam: CPT 92004 (new) and 92014 (established) — general examination of the complete visual system that always includes initiation of diagnostic and treatment programs; there must be initiation of treatment or a diagnostic plan to report a comprehensive service.

Clinical requirementPatient must need and receive care for a condition other than refractive error or routine screening; follow-up that does not require diagnosis or treatment does not qualify.

Examples not qualifyingVisits primarily for refraction or routine screening (e.g., preventive exam, early asymptomatic cataract seen incidentally) should not be reported with 92004/92014 unless diagnostic/treatment initiation occurs.

Routine ophthalmological evaluation (S0620/S0621)
